Baş Bölgesinin Yumuşak Doku Hastalıkları
Özet
Bu çalışma, evcil hayvanlarda baş ve boyun bölgesinde şekillenen yumuşak doku hastalıklarını etiyoloji, patogenez, klinik görünüm, tanı ve güncel sağaltım prensipleri açısından kapsamlı bir şekilde incelemektedir. Bölümün ilk kısmında, flegmon ve apselerin ayırıcı özellikleri ele alınarak; bu lezyonların polimikrobiyal doğası, görüntüleme yöntemlerindeki (MRG, USG) tipik bulguları ve cerrahi drenaj teknikleri detaylandırılmaktadır. Baş ve boyun bölgesi neoplazileri başlığı altında; sığırlarda oküler skuamöz hücreli karsinom, atlarda sarkoidler ve kedi/köpeklerdeki oral tümörlerin biyolojik davranışları ile Bilgisayarlı Tomografi (BT) destekli modern tanı yaklaşımları irdelenmektedir. Nöromüsküler patolojiler kapsamında, yedinci kranial sinir (Nervus Facialis) felcinin patofizyolojisi, Seddon ve Sunderland sınıflaması üzerinden açıklanmakta; idiyopatik fasiyal nöropati ve otitis kaynaklı paralizilerin klinik yönetimi sunulmaktadır. Beşinci kranial sinir (Nervus Trigeminus) disfonksiyonu ile karakterize "düşük çene" tablosu ve bu durumun Çiğneme Kası Miyozitisi (MMM) ile ayırıcı tanısı, 2M antikor testi ve EMG gibi spesifik yöntemler ışığında tartışılmaktadır. Ayrıca, özellikle brakisefalik köpek ırklarında yaygın görülen Dudak Kıvrımı Piyoderması (Intertrigo) ve cerrahi çözüm yolu olan şiloplasti operasyonuna yer verilmektedir. Çalışmanın son bölümünde submandibular bölgenin lenfatik ve derin doku hastalıkları; juvenil sellülitis, lenfadenitisler ve sığırlarda kronik karakterli "soğuk apseler" (aktinomikoz, aktinobasilloz) üzerinden analiz edilmektedir. Baş bölgesinin karmaşık anatomisi ve hayati yapılara yakınlığı göz önüne alındığında, bu patolojilerin yönetiminde multidisipliner bir yaklaşımın ve doğru ayırıcı tanının prognoz üzerindeki kritik önemi vurgulanmaktadır.
