Respiratory System Diseases in Children
Özet
Bu bölümde, çocuklarda görülen solunum sistemi bozuklukları ayrıntılı olarak ele alınmaktadır. Çocukların solunum sistemi hem anatomik hem de fizyolojik açıdan yetişkinlerinkinden farklıdır; hava yollarının darlığı, alveollerin tam olarak gelişmemiş olması ve sınırlı solunum rezervi nedeniyle çocukların enfeksiyonlara ve solunum yetmezliğine daha yatkın olduğu vurgulanmaktadır. Solunum sistemi üst ve alt solunum yolları açısından incelenmekte ve çocuklarda gaz değişimi ile solunum mekanizması ve metabolik gereksinimler arasındaki ilişki açıklanmaktadır. Ayrıca fizik muayene, solunum seslerinin değerlendirilmesi, oksijen satürasyonu, göğüs röntgeni ve solunum fonksiyon testleri gibi tanı yöntemlerinin önemi vurgulanmaktadır. Bu bölümde, akut rinofarenjit, farenjit, tonsillit, krup, sinüzit, larenjit ve akut orta kulak iltihabı dahil olmak üzere üst solunum yolu hastalıkları ayrıntılı olarak açıklanmaktadır. Bu hastalıkların çoğunun viral kökenli olduğu, çocuklarda yaygın olduğu ve tipik olarak öksürük, ateş, burun akıntısı, boğaz ağrısı ve solunum sıkıntısı gibi semptomlarla ortaya çıktığı belirtilmektedir. Alt solunum yolu hastalıkları başlığı altında bronşiyolit, pnömoni, astım, bronşit, kistik fibroz ve tüberküloz ele alınmaktadır. Bronşiyolitin bebeklerde yaygın olduğu, astımın ise çocukluk çağının en yaygın kronik hastalığı olduğu özellikle belirtilmektedir. Bu durumlarda erken tanı, oksijen desteği, uygun ilaç tedavisi, hidrasyonun sağlanması ve enfeksiyon kontrolü temel yaklaşımlar arasındadır. Bölümün önemli bir kısmı hemşirelik bakımına ayrılmıştır. Çocuk ve aile merkezli bakım yaklaşımının önemi vurgulanmaktadır; solunumun izlenmesi, hava yolunun açık tutulması, uygun pozisyonlandırma, aile eğitimi ve enfeksiyon kontrolü, hemşirelik bakımının temel unsurları olarak belirlenmiştir. Güncel yaklaşımlar arasında non-invaziv ventilasyon teknikleri, evde bakım modelleri ve dijital izleme sistemlerinin kullanımının arttığına dikkat çekilmektedir.
