Yabancı Cisim Lenfadenopati
Özet
Bu kitap bölümünde yabancı cisim lenfadenopatisinin tanımı, patogenezi, klinik önemi, histopatolojik özellikleri ve ayırıcı tanısı ele alınmaktadır. Yabancı cisim lenfadenopatisi; eksojen veya endojen materyallerin lenfatik drenaj yoluyla lenf nodlarına taşınması ve histiyositik/yabancı cisim tipi inflamatuvar yanıt oluşturmasıyla gelişen reaktif bir lenf nodu patolojisidir. Bölümde proteinöz lenfadenopati, silikon lenfadenopatisi, eklem protezi ilişkili lenfadenopati, altın lenfadenopatisi, lipid lenfadenopatisi ve cerrahi materyal, dövme pigmenti, silika, kömür tozu gibi diğer yabancı cisim kaynaklı lenfadenopatiler sistematik olarak incelenmektedir. Her alt başlıkta etiyolojik faktörler, klinik yansımalar, mikroskobik bulgular, immünohistokimyasal özellikler ve tanısal ipuçları özetlenmiştir. Özellikle silikon implant, protez materyali veya malignite öyküsü bulunan hastalarda bu lezyonların metastatik karsinom, lenfoma, granülomatöz enfeksiyonlar ve diğer reaktif lenfadenopatilerle karışabileceği vurgulanmaktadır. Doğru tanı için klinik öykü, histopatolojik morfoloji, özel boyalar, polarize ışık incelemesi ve gerektiğinde ek analitik yöntemlerin birlikte değerlendirilmesi gereklidir.
This book chapter discusses the definition, pathogenesis, clinical significance, histopathological features, and differential diagnosis of foreign body lymphadenopathy. Foreign body lymphadenopathy is a reactive lymph node pathology that develops when exogenous or endogenous materials are transported to lymph nodes via lymphatic drainage, triggering a histiocytic or foreign body–type inflammatory response. The chapter systematically reviews proteinaceous lymphadenopathy, silicone lymphadenopathy, prosthesis-associated lymphadenopathy, gold lymphadenopathy, lipid lymphadenopathy, and other foreign body–related lymphadenopathies caused by surgical materials, tattoo pigments, silica, and coal dust. For each entity, etiological factors, clinical manifestations, microscopic findings, immunohistochemical characteristics, and diagnostic clues are summarized. Particular emphasis is placed on the potential for these lesions to mimic metastatic carcinoma, lymphoma, granulomatous infections, and other reactive lymphadenopathies, especially in patients with a history of silicone implants, prosthetic materials, or malignancy. Accurate diagnosis requires comprehensive evaluation of clinical history, histopathological morphology, special stains, polarized light examination, and, when necessary, additional analytical methods.
