Diş Çekimi ve Cerrahi Teknikler
Özet
Diş çekimi (ekzodonti), kedi ve köpeklerde ağız ve diş hastalıklarının tedavisinde yaygın olarak uygulanan ve klinik karar süreci uygun şekilde yönetilmediğinde ciddi komplikasyonlara yol açabilen cerrahi bir girişimdir. Veteriner diş hekimliğinde temel yaklaşım dişin korunması olmakla birlikte; periodontal hastalıklar, diş rezorpsiyonları, felin kronik gingivostomatitis, periapikal enfeksiyonlar ve maloklüzyonlar gibi durumlarda ekstraksiyon kaçınılmaz hâle gelebilmektedir. Bu bölümde, diş çekimine karar verme sürecinden başlayarak basit ve cerrahi ekstraksiyon teknikleri sistematik ve klinik uygulamaya yönelik bir yaklaşımla ele alınmıştır. Detaylı ağız muayenesi ve intraoral radyografinin tanısal önemi ile uygun alet-ekipman seçimi ve preemptif multimodal analjezinin cerrahi başarı üzerindeki etkileri vurgulanmıştır. Ayrıca kök kırıkları, kanama, oronazal fistül, mandibular kırıklar ve oftalmik komplikasyonlar gibi ekstraksiyona bağlı gelişebilecek komplikasyonlar ile bunların önlenmesi ve yönetimine ilişkin temel cerrahi prensipler sunulmuştur. Sonuç olarak başarılı ve güvenli bir diş ekstraksiyonu; doğru endikasyonun belirlenmesi, uygun tekniklerin uygulanması ve yeterli anatomik bilgi ile cerrahi deneyime dayanan dikkatli bir yaklaşım gerektirir.
Tooth extraction (exodontia) is a commonly performed surgical procedure in the treatment of oral and dental diseases in cats and dogs and may lead to serious complications when the clinical decision-making process is not managed appropriately. Although the primary approach in veterinary dentistry is preservation of teeth, extraction may become unavoidable in conditions such as periodontal disease, tooth resorption, feline chronic gingivostomatitis, periapical infections, and malocclusions. In this chapter, the decision-making process for tooth extraction, as well as simple and surgical extraction techniques, are discussed in a systematic and clinically oriented manner. The diagnostic importance of detailed oral examination and intraoral radiography, together with the effects of appropriate instrument selection and preemptive multimodal analgesia on surgical success, are emphasized. In addition, major complications associated with tooth extraction, including root fractures, hemorrhage, oronasal fistula, mandibular fractures, and ophthalmic complications, along with the basic surgical principles for their prevention and management, are presented. In conclusion, successful and safe tooth extraction requires appropriate case selection, proper surgical techniques, and a careful approach based on sufficient anatomical knowledge and surgical experience.
