Mesleki Etik İlkeler Ebelerin Hakları ve Sorumlulukları
Özet
Saygılı annelik bakımı, gebelikten doğum sonu döneme uzanan süreçte kadınların onurunun, mahremiyetinin ve özerkliğinin korunmasını esas alan, insan hakları ve etik ilkelere dayalı bir bakım yaklaşımıdır. Dünya Sağlık Örgütü ve Uluslararası Ebeler Konfederasyonu tarafından temel bir insan hakkı olarak ele alınan bu yaklaşım, kadınların karar verme süreçlerine etkin katılımını desteklemekte ve doğum deneyimlerinin niteliğini doğrudan etkilemektedir. Ebeler, doğum hizmetlerinde kadınla en yakın ve sürekli ilişki kuran sağlık profesyonelleri olarak saygılı annelik bakımının uygulanmasında merkezi bir role sahiptir. Ebelik etik kodları, insan onuruna saygı, zarar vermeme, yarar sağlama, adalet, eşitlik, mahremiyetin korunması, doğruluk ve bilgilendirilmiş onam gibi temel ilkeler doğrultusunda bakımın çerçevesini belirlemektedir. Bununla birlikte, saygılı annelik bakımının sürdürülebilirliği yalnızca bireysel mesleki tutumlarla sınırlı değildir. Yetersiz insan gücü, yoğun iş yükü, fiziksel altyapı eksiklikleri, mesleki özerkliğin sınırlanması ve etik konusunda eğitim erişimindeki eşitsizlikler, ebelerin etik sorumluluklarını yerine getirmesini zorlaştırmaktadır. Bu nedenle saygılı annelik bakımının güçlendirilmesi, ebelerin mesleki haklarını gözeten, destekleyici çalışma koşullarını ve etik duyarlılığı kurumsal düzeyde destekleyen sağlık sistemleriyle birlikte ele alınmalıdır.
Respectful maternity care is an approach grounded in human rights and ethical principles that prioritizes the protection of women’s dignity, privacy, and autonomy throughout the continuum from pregnancy to the postpartum period. Recognized as a fundamental human right by the World Health Organization and the International Confederation of Midwives, this approach supports women’s active participation in decision-making processes and directly influences the quality of the childbirth experience. Midwives, as the healthcare professionals who establish the closest and most continuous relationship with women during maternity care, play a central role in the implementation of respectful maternity care. Midwifery codes of ethics provide a framework for care based on core principles such as respect for human dignity, non-maleficence, beneficence, justice, equity, protection of privacy, veracity, and informed consent. However, the sustainability of respectful maternity care cannot be limited to individual professional attitudes alone. Insufficient staffing, high workload, inadequacies in physical infrastructure, restrictions on professional autonomy, and inequalities in access to ethics-related education hinder midwives’ ability to fulfill their ethical responsibilities. Therefore, strengthening respectful maternity care requires health systems that acknowledge midwives’ professional rights and support ethical sensitivity through enabling working conditions at the institutional level.
