Endodontik-Periodontal Lezyonlar: Moleküler Patobiyoloji, Tanısal Algoritma ve Prognoz Temelli Klinik Yönetim
Özet
Bu bölüm, endodontik–periodontal lezyonların biyolojik temellerini, tanısal değerlendirme sürecini ve prognoz temelli klinik yönetimini bütüncül bir yaklaşımla ele almaktadır. Pulpa ile periodonsiyum arasındaki ilişkinin apikal foramen, lateral ve aksesuar kanallar ile dentinal tübüller aracılığıyla kurulduğu; bu anatomik yolların enfeksiyonun iki kompartıman arasında yayılmasına olanak sağladığı açıklanmaktadır. Ayrıca perforasyonlar, vertikal kök kırıkları ve rezorptif lezyonlar gibi yapısal bozuklukların pulpa–periodontal bariyeri bozarak prognozu belirgin biçimde etkileyebileceği tartışılmaktadır. Bölümde IL-1β, TNF-α ve RANKL aracılı inflamasyon–kemik rezorpsiyonu mekanizmaları özetlenmekte; bu moleküler süreçlerin klinik bulgularla ilişkisi açıklanmaktadır. Tanısal yaklaşımda pulpa vitalite testleri, periodontal sondlama paterni, fistül yolunun radyografik izlenmesi ve seçici konik ışınlı bilgisayarlı tomografi kullanımının önemi vurgulanmaktadır. Tedavi planlamasında enfeksiyon kaynağının doğru belirlenmesi, endodontik tedavinin önceliklendirilmesi ve periodontal müdahalelerin biyolojik iyileşme potansiyeline göre planlanması gerektiği belirtilmektedir. Bu çerçevede bölüm, endodontik–periodontal lezyonların tanı, tedavi sıralaması ve prognoz değerlendirmesinde klinisyenlere sistematik ve kanıta dayalı bir yaklaşım sunmaktadır.
This chapter provides a comprehensive overview of endodontic–periodontal lesions by integrating molecular pathobiology, diagnostic strategies, and prognosis-based clinical management. It describes the biological relationship between the pulp and the periodontium, emphasizing anatomical communication pathways such as the apical foramen, lateral and accessory canals, and dentinal tubules that facilitate the spread of infection between these compartments. Structural defects including perforations, vertical root fractures, and resorptive lesions are also discussed as critical factors that compromise the pulp–periodontal barrier and significantly influence prognosis. The chapter outlines inflammatory and bone resorption mechanisms mediated by IL-1β, TNF-α, and RANKL, linking molecular events with clinical manifestations. Diagnostic assessment focuses on the combined interpretation of pulp vitality tests, periodontal probing patterns, sinus tract tracing, and the selective use of cone-beam computed tomography. In terms of treatment planning, priority is given to accurate identification of the primary infection source, appropriate sequencing of endodontic and periodontal interventions, and evaluation of biological healing potential. Overall, the chapter offers clinicians a structured, evidence-based framework for diagnosis, treatment sequencing, and prognostic assessment of endodontic–periodontal lesions.
