Periodontolojide Platelet Rich Fibrin (PRF) Kullanımı
Özet
Platelet-rich fibrin (PRF), son yıllarda diş hekimliğinde yara iyileşmesini biyolojik olarak destekleyen otojen bir biyomateryal olarak giderek artan ilgi görmektedir. Antikoagülan veya dış aktivatör gerektirmeden hastanın kendi kanından elde edilmesi ve üç boyutlu fibrin matriksi içerisinde trombositler, lökositler ve çeşitli büyüme faktörlerini barındırması PRF’i klinik açıdan değerli bir biyolojik ajan haline getirmiştir. Bu bölümde PRF’in tarihsel gelişimi, biyolojik temeli, etki mekanizmaları ve hazırlanma protokolleri ayrıntılı biçimde ele alınmıştır. Ayrıca PRF türevleri, fibrin membranlarının özellikleri ve farklı biyomateryaller ile etkileşimleri tartışılmıştır. Bölümün klinik kısmında PRF’in periodontoloji, implantoloji ve oral cerrahi alanlarındaki kullanım endikasyonları güncel literatür ışığında değerlendirilmiştir. Mevcut kanıtlar, PRF’in özellikle yumuşak doku iyileşmesi, postoperatif konfor ve erken yara stabilitesi üzerinde olumlu etkiler sağlayabildiğini göstermektedir. Bununla birlikte PRF’in sert doku rejenerasyonunda tek başına belirleyici bir materyal olmadığı ve daha çok biyolojik destekleyici bir ajan olarak değerlendirilmesi gerektiği vurgulanmaktadır. Sonuç olarak PRF, doğru endikasyon ve uygun cerrahi tekniklerle birlikte kullanıldığında periodontoloji ve oral cerrahi pratiğinde önemli bir biyolojik destekleyici materyal olarak değerlendirilebilir.
Platelet-rich fibrin (PRF) has gained increasing attention in dentistry as an autologous biomaterial that biologically supports wound healing. PRF is obtained from the patient’s own blood without the use of anticoagulants or external activators and contains platelets, leukocytes, and multiple growth factors within a three-dimensional fibrin matrix. These characteristics make PRF a biologically active and clinically valuable material in regenerative dentistry. This chapter discusses the historical development of PRF, its biological basis, mechanisms of action, and preparation protocols. In addition, PRF derivatives, characteristics of PRF membranes, and their interactions with other biomaterials are addressed. The clinical applications of PRF in periodontology, implant dentistry, and oral surgery are evaluated based on current scientific evidence. Available data indicate that PRF may positively influence soft tissue healing, postoperative comfort, and early wound stabilization. However, PRF should not be considered a standalone regenerative material for hard tissue regeneration and is better regarded as a biological adjunct that supports surgical outcomes. When used with appropriate indications and surgical techniques, PRF can serve as an important biological adjunct in periodontal and oral surgical procedures.
