Kasıtlı Reimplantasyon
Özet
Kasıtlı reimplantasyon, cerrahi olmayan yeniden endodontik tedavi veya apikal mikrocerrahinin uygulanamadığı ya da başarısız olduğu olgularda, hastanın kendi dişinin korunmasını amaçlayan biyolojik temelli bir tedavi yaklaşımıdır. Planlı ve kontrollü bir prosedür olarak uygulanır. Bu yöntemde diş, atravmatik bir şekilde çekilir; ağız dışında gerekli endodontik işlemler yapıldıktan sonra kısa süre zarfında kendi alveol soketine tekrar yerleştirilir. Tedavinin başarısı; uygun vaka seçimi, periodontal ligament hücre canlılığının korunması, ekstraoral sürenin olabildiğince kısa tutulması ve kök ucu dolgusunun biyouyumlu materyallerle sızdırmaz biçimde gerçekleştirilmesi gibi temel biyolojik ve klinik prosedürlere bağlıdır. Güncel literatür, doğru endikasyon ve modern mikrocerrahi prensipler doğrultusunda uygulandığında kasıtlı reimplantasyonun yüksek başarı ve sağkalım oranlarına ulaşabildiğini göstermektedir. Dental mikroskopların kullanımı, ultrasonik kök ucu hazırlığı teknikleri ve MTA ile biyoseramik esaslı materyallerin klinik pratiğe girmesi, bu yöntemin öngörülebilirliğini artırmıştır. Ayrıca periodontal ligamentin korunması sayesinde propriosepsiyonun devamlılığı ve alveoler kemik bütünlüğünün sürdürülmesi, implant tedavilerine kıyasla önemli biyolojik avantajlar sunmaktadır. Bununla birlikte ankiloz ve kök rezorpsiyonu gibi olası komplikasyonlar dikkatle izlenmelidir. Sonuç olarak kasıtlı reimplantasyon, implant öncesi son koruyucu basamak olarak değerlendirilebilen, uygun hasta ve doğru teknikle uygulandığında klinik olarak kabul edilebilir ve biyolojik açıdan avantajlı bir tedavi seçeneğidir.
Intentional replantation is a biologically grounded treatment modality aimed at preserving the natural tooth in cases where nonsurgical endodontic retreatment or apical microsurgery is not feasible or has previously failed. It is performed as a deliberate and carefully controlled procedure. In this approach, the tooth is extracted atraumatically, the required endodontic procedures are completed extraorally, and the tooth is subsequently repositioned into its original alveolar socket as rapidly as possible. Treatment success depends on several key biological and clinical factors, including appropriate case selection, preservation of periodontal ligament cell viability, minimization of extraoral time, and the provision of a well-sealed root-end filling using biocompatible materials. Contemporary evidence suggests that, when performed under appropriate indications and in accordance with modern microsurgical principles, intentional replantation can yield high success and survival rates. The use of dental operating microscopes, ultrasonic root-end preparation techniques, and contemporary materials such as mineral trioxide aggregate (MTA) and bioceramic-based cements has further improved the predictability of this procedure. Moreover, preservation of the periodontal ligament offers important biological advantages over implant therapy, including maintenance of proprioception and preservation of alveolar bone integrity. Nevertheless, potential complications such as ankylosis and root resorption should be carefully monitored during follow-up. In conclusion, intentional replantation represents a clinically acceptable and biologically advantageous treatment option and may be considered the final tooth-preserving intervention before implant therapy in properly selected patients when performed with appropriate technique.
