Multipl Sklerozda Afet Yönetimi ve Hemşirelik Yaklaşımı
Özet
Afetler, hem ülkemizde hem de dünyada sıklığı artan ve sağlık hizmetlerinin sürekliliğini kesintiye uğratarak birey ve toplum üzerinde ciddi etkiler oluşturan olağanüstü durumlardır. Bu süreçler, özellikle kronik hastalığı olan bireyler için önemli riskler barındırmaktadır. Multipl Skleroz (MS), merkezi sinir sistemini etkileyen, ataklarla seyreden kronik ve ilerleyici bir nörolojik hastalıktır. MS’li bireylerde mobilite, günlük yaşam aktiviteleri ve fonksiyonel bağımsızlıkta görülen kısıtlılıklar, afet koşullarında bakım ve izlem süreçlerini daha da güçleştirmektedir. Afet süreçlerinde tedaviye ve ilaçlara erişimin güçleşmesi, mobilite kısıtlılığı nedeniyle tahliyede yaşanan zorluklar ve çevresel ısı değişimlerinin semptomları tetiklemesi gibi sorunlar ortaya çıkabilmektedir. Ayrıca afetin yarattığı stres, belirsizlik ve olumsuz çevresel koşullar hastalığın seyrini olumsuz yönde etkileyebilmektedir. Bu nedenle hemşirelik rolü yalnızca bakım sunumuyla sınırlı olmayıp; afet öncesi hazırlık, afet anı yönetimi ve afet sonrası rehabilitasyonu kapsayan bütüncül bir yaklaşımı gerektirmektedir. Literatürde MS ve afet yönetimi ilişkisini ele alan çalışmaların sınırlı olduğu, ancak kronik hastalığa sahip bireylerin afet koşullarında daha kırılgan hale geldiğinin vurgulandığı görülmektedir. Afetlerde MS’li bireylerin bakımında hemşireler; hazırlık, müdahale ve iyileşme aşamalarında kritik sorumluluklar üstlenmektedir. Bu alandaki bilgi ve farkındalığın artırılması, afetlerin MS’li bireyler üzerindeki olumsuz etkilerinin azaltılmasında belirleyici öneme sahiptir. Bu bölümün amacı, multipl sklerozlu bireylerde afet yönetimini ve hemşirelik yaklaşımını kapsamlı biçimde ele almaktır.
Disasters are extraordinary events that have become increasingly frequent both in our country and worldwide, disrupting the continuity of healthcare services and exerting profound effects on individuals and communities. These situations pose particularly significant risks for individuals living with chronic illnesses. Multiple sclerosis (MS) is a chronic, progressive neurological disorder affecting the central nervous system and characterized by relapsing episodes. In individuals with MS, limitations in mobility, activities of daily living, and functional independence further complicate care and monitoring processes under disaster conditions. During disasters, barriers to accessing treatment and medications, evacuation difficulties related to mobility impairments, and the exacerbation of symptoms due to environmental temperature fluctuations may arise. Moreover, stress, uncertainty, and adverse environmental circumstances associated with disasters can negatively influence the course of the disease. Therefore, the nursing role extends beyond direct care provision and requires a comprehensive, holistic approach encompassing pre-disaster preparedness, disaster response, and post-disaster rehabilitation. Although the literature addressing the intersection of MS and disaster management remains limited, existing evidence underscores the heightened vulnerability of individuals with chronic conditions in disaster settings. Nurses assume critical responsibilities in the preparedness, response, and recovery phases of disaster management for individuals with MS. Enhancing knowledge and awareness in this area is pivotal to mitigating the adverse effects of disasters on this population. The aim of this chapter is to comprehensively examine disaster management and the nursing approach in individuals living with multiple sclerosis.
