Yaygın İntrakraniyal Kanama Nedeniyle Beyin Ölümü Gerçekleşen ve Organ Donasyonuna Giden Hastada Donör Bakımı Yönetimi
Özet
58 yaşında erkek hasta, hipertansiyon, koroner arter hastalığı ve benign prostat hiperplazisi öyküsüyle evde aniden gelişen bilinç kaybı ve gasping solunum nedeniyle acil servise getirildi. İlk değerlendirmede Glasgow Koma Skoru 3, bilateral midriazis, beyin sapı reflekslerinin kaybı ve hipertansif kriz (220/130 mmHg) saptandı. Hastaya hızlı ardışık entübasyon uygulandı. Beyin bilgisayarlı tomografide ventriküllere açılan, WFNS (World Federation of Neurological Surgeons) skoru 5 düzeyinde ağır subaraknoid kanama görüldü. Acil olarak eksternal ventriküler drenaj yerleştirildi ve hasta yoğun bakım ünitesine alındı. Yoğun bakımda yapılan seri nörolojik değerlendirmelerde spontan solunumun ve beyin sapı reflekslerinin yokluğu nedeniyle beyin ölümü ön tanısı konuldu. Apne testi sırasında PaCO₂ artışına rağmen solunum çabası izlenmedi. Nörolojik muayeneler, EEG ve kontrast anjiyografi ile 3. gün beyin ölümü tanısı kesinleşti. Aileden alınan onam sonrasında organ bağışı süreci başlatıldı. Donör yönetimi kapsamında hemodinamik destek amacıyla noradrenalin infüzyonu, sıvı-elektrolit dengesinin sağlanması ve santral diabetes insipidus için IV desmopressin uygulandı. Hormon replasman tedavisi (levotiroksin, hidrokortizon, vazopressin) başlandı. Mekanik ventilasyonda akciğer koruyucu stratejiler uygulandı, enfeksiyon taraması gerçekleştirildi. Multidisipliner transplant ekibi değerlendirmesinde kalp, karaciğer, böbrekler, kornea ve akciğerlerin transplantasyon için uygun olduğu belirlendi. Karaciğer, akciğerler, kornea ve iki böbreğin alıcılara nakli başarılı şekilde gerçekleştirildi.
