Epidural Anestezi Sonrası Gelişen Lokal Anestezik Sistemik Toksisitesi: Olgu Sunumu-Lokal Anestezik Sistemik Toksisitesi Patofizyoloji, Klinik Bulgular ve Yönetim Yaklaşımları
Özet
Bilinen ek hastalık öyküsü olmayan, 21 yaşında erkek hasta, çoklu alt ekstremite fraktürleri nedeniyle operasyon öyküsü olan hasta, kemik ekspozisyonu üzerine estetik rekonstruktif ve plastik cerrahi kliniği tarafından serbest flep rekonstrüksiyonu yapılmak üzere vakaya alındı.Epidural ve genel anestezi altında, epidural bupivakain uygulaması sonrası lokal anestezik sistemik toksisitesi (LAST) gelişti ve şiddetli hipotansiyon izlendi. Efedrin, inotrop ajanlar (dobutamin, dopamin, norepinefrin) ve iki kez uygulanan %20 lipid emülsiyonu ile normotansiyon sağlandı. Operasyon süresince masif sıvı ve kan replasmanı yapıldı. Yoğun bakımda yakın takip edilen hastada nörojenik şok ve kardiyak patoloji ekarte edildi, antibiyoterapi başlandı. 30. saatte ekstübe edilen hasta, 72. saatte inotrop desteği kesildikten sonra stabil vitaller ve GKS 15 ile 80. Saatte estetik renkonstruktif ve plastik cerrahi servisine devredildi. Lokal anestezikler (LA), cerrahi ve invaziv girişimlerde ağrıyı bloke etmek amacıyla yaygın olarak kullanılmaktadır. Ancak sistemik toksisite, özellikle yanlış uygulama, doz aşımı veya ilacın istemsiz intravenöz enjeksiyonu sonrasında ciddi komplikasyonlara yol açabilir. Lokal anestezik sistemik toksisitesi (LAST), merkezi sinir sistemi ve kardiyovasküler sistem üzerinde potansiyel olarak hayatı tehdit edici etkiler oluşturabilir. Lokal anestezikler (LA) anestezi klinisyenleri, cerrahlar, acil servis sağlayıcıları, diş hekimleri dahil olmak üzere birçok klinisyen tarafından pratikte uygulanmaktadır. LA'ların yaygın kullanımına rağmen, lokal anestezik sistemik toksisitesine (LAST) ilişkin farkındalık ve bunun yönetimine ilişkin bilgi eksikliği mevcuttur (1,2). Bu çalışmada lokal anestezik toksisitesinin patofizyolojisi, klinik belirtileri, tanı yöntemleri ve tedavi yaklaşımları güncel literatür ışığında incelenmiştir.
