Şilotoraks Gelişen Vakada Yoğun Bakım Yönetimi: Olgu Sunumu
Özet
20 yaş erkek hasta araç dışı trafik kazası ve kafa travması nedeni ile takipli iken dispne, taşikardi, oksijen saturasyon düşüklüğü gelişti. Göğüs x-ray görüntülemede subtotal sağ plevral efüzyon geliştiği tespit edildi.Plevral effüzyon örneği süt rengindeydi. Effüzyondan gönderilen örnekte trigliserit düzeyi yüksek(>110 mg dl) kolesterol seviyesi düşük(<200 mg dl) tespit edildi. Şilotraks tanısı konulan hasta göğüs tüp drenajı uygulandı. Hastanın enteral beslenmesi kesildi, parenteral beslenmeye geçildi.Somatostatin 6 gr/gün 24 saat sürekli intravenöz infüzyon tedavisi başlandı.Takiplerinde efüzyonda düzelme izlendi. Göğüs tüpü çekildi. Yoğun bakım tedavisi tamamlanan hasta servise devir edildi.
A 20-year-old male patient developed dyspnea, tachycardia, and low oxygen saturation while being followed up for a traffic accident and head trauma outside the vehicle. A chest x-ray revealed a subtotal right pleural effusion. The pleural effusion sample was milky in color. The sample sent from the effusion revealed high triglyceride levels (>110 mg dL) and low cholesterol levels (<200 mg dL). The patient was diagnosed with chylothrax, and chest tube drainage was performed. Enteral feeding was discontinued, and parenteral nutrition was initiated. Somatostatin 6 g/day continuous 24-hour intravenous infusion therapy was initiated. Improvement in the effusion was observed during follow-up. The chest tube was removed. Following completion of intensive care treatment, the patient was transferred to the ward.
