Vücut İçi Uyuşturucu Taşıyıcılığı Olgusunda Anestezi ve Yoğun Bakım Yaklaşımı
Özet
Vücut içi uyuşturucu taşıyıcılığı, ciddi toksisite ve ölüm riski taşıyan, acil ve yoğun bakım pratiğinde giderek daha sık karşılaşılan bir klinik tablodur. Bu bölümde, özellikle metamfetamin içeren paketlerin gastrointestinal sistemde taşınmasına bağlı gelişebilecek komplikasyonlar ve bu olgulara yönelik anestezi ve yoğun bakım yaklaşımları ele alınmaktadır. Tanıda direkt batın grafisi ve bilgisayarlı tomografinin önemi vurgulanırken, semptomatik ve asemptomatik hastaların yönetimindeki farklılıklar tartışılmaktadır. Sunulan olgu üzerinden, semptom göstermeyen bir vücut içi taşıyıcının konservatif yöntemler ve yakın yoğun bakım izlemi ile başarılı şekilde yönetilebileceği gösterilmektedir. Bölüm, multidisipliner yaklaşımın ve özellikle kardiyovasküler komplikasyonlar açısından dikkatli izlemin hayati önemini ortaya koymaktadır.
Internal drug concealment, also known as body packing, represents a life-threatening condition frequently encountered in emergency and intensive care settings. This chapter focuses on anesthetic and intensive care management of patients who internally transport illicit drugs, particularly methamphetamine-containing packets. The diagnostic value of plain abdominal radiography and computed tomography is emphasized, along with management strategies for both symptomatic and asymptomatic patients. Through a presented case, the chapter demonstrates that asymptomatic body packers can be successfully managed with conservative treatment and close intensive care monitoring. The importance of early diagnosis, avoidance of unnecessary invasive procedures, and a multidisciplinary approach is highlighted. Special attention is given to the risk of cardiovascular complications associated with sympathomimetic agents such as methamphetamine.
