Hirudoterapi (Sülük Terapisi)’Nin Cerrahi Bakımda Kullanımı
Özet
Hirudoterapi, tıbbi sülüklerin biyolojik aktif bileşenleri aracılığıyla sağlığa olumlu etkiler sunduğu bir geleneksel ve tamamlayıcı tıp uygulamasıdır. Tarihsel olarak antik çağlardan itibaren çeşitli hastalıkların düzeltilmesinde kullanılan bu yöntem, günümüzde cerrahi sonrası dolaşım bozuklukları, venöz konjesyon ve rekonstrüktif cerrahi gibi alanlarda uygulanmaktadır. Hirudoterapinin etkinliğini sağlayan enzimler arasında hirudin, hiyalüronidaz ve kollajenaz gibi bileşenler yer almakta olup, bu maddeler antikoagülan, antiinflamatuar ve analjezik etkiler göstermektedir. Ancak, enfeksiyon, aşırı kanama ve alerjik reaksiyon gibi yan etkiler gelişebileceğinden, hirudoterapinin uygulanmaması gereken durumlar dikkatle değerlendirilmelidir. Cerrahi hastalarında bu yöntemin güvenli ve etkili şekilde uygulanabilmesi için hemşirelerin rolü kritiktir. Hemşireler, hasta izlemi, olası komplikasyonların önlenmesi ve hasta eğitimi gibi süreçlerde önemli sorumluluklar üstlenmektedir. Bu kitap bölümü, cerrahide hirudoterapinin kullanım alanlarını, uygulanma yöntemlerini ve hemşirelik bakımına yönelik temel ilkeleri ele alarak, konuya kapsamlı bir bakış açısı sunmayı amaçlamaktadır.
Hirudotherapy is an traditional and complementary treatment method that provides health benefits through the biologically active components of medicinal leeches. Historically used since antiquity for various diseases, this method is currently applied in areas such as postoperative circulatory disorders, venous congestion, and reconstructive surgery. The effectiveness of hirudotherapy is attributed to enzymes such as hirudin, hyaluronidase, and collagenase, which exhibit anticoagulant, anti-inflammatory, and analgesic effects. However, as adverse effects such as infections, excessive bleeding, and allergic reactions may occur, contraindications for hirudotherapy should be carefully assessed. Nurses play a critical role in ensuring the safe and effective application of this method in surgical patients. They are responsible for patient monitoring, preventing potential complications, and providing patient education. This book chapter aims to offer a comprehensive perspective on the use of hirudotherapy in surgical patients, its application methods, and fundamental principles of nursing care.
