Biyogeribildirimin Cerrahi Bakımda Kullanımı
Özet
Biyogeribildirim; hasta bireyin kalp hızı değişkenliği, elektromiyografi, deri iletkenliği gibi fizyolojik sinyallerini gerçek zamanlı görselleştirerek kendi fizyolojik yanıtlarını düzenlemeyi öğreten, girişimsel olmayan bir geleneksel ve tamamlayıcı tıp uygulamasıdır. Cerrahi bakım ortamında biyogeribildirim, ameliyat öncesi anksiyetenin giderilmesi, ameliyat sonrası ağrı yönetimi, solunum rehabilitasyonu ve fonksiyonel iyileşme gibi çeşitli hedeflerle kullanılmıştır. Son yıllarda kalp hızı değişkenliği bazlı yaklaşımlar ve elektromiyografi bazlı geri bildirimler ile biyogeribildirim destekli sanal gerçeklik uygulamaları klinik çalışmalarda incelenmiştir. Örneğin; pelvik taban ameliyat öncesi biyogeribildirim çalışmalarında idrar kaçırma ve kontinans geri kazanımı gibi spesifik kazanımlar meta-analizlerde ortaya konulmuştur. Bununla birlikte çalışmaların heterojenliği, örneklem büyüklükleri ve uygulama protokollerindeki farklılıklar nedeniyle kanıt düzeyleri arasında değişiklik görülmektedir. Bu bölümde cerrahi bakımda (ameliyat öncesi, sırası ve sonrası) biyogeribildirim uygulamaları, etki mekanizmaları, güncel klinik çalışmalar ve bakıma entegrasyon örnekleri özetlenmiştir.
Biofeedback is a non-invasive traditional and complementary medical practice that teaches patients to regulate their own physiological responses by visualizing real-time physiological signals (such as heart rate variability, electromyography, and skin conductance). In surgical care settings, biyogeribildirim has been used for various purposes, including reducing preoperative anxiety, managing postoperative pain, supporting respiratory rehabilitation, and promoting functional recovery. In recent years, heart rate variability–based approaches, electromyography-based feedback, and biyogeribildirim-assisted virtual reality applications have been examined in clinical studies. For example meta-analyses of preoperative pelvic floor biyogeribildirim interventions have demonstrated specific benefits such as reduction of urinary incontinence and improvement of continence. However, due to heterogeneity among studies, sample sizes, and differences in intervention protocols, the levels of evidence vary. This chapter summarizes biyogeribildirim applications in surgical care (preoperative, intraoperative, and postoperative), their mechanisms of action, current clinical research, and examples of integration into care.
