Deneysel Nöropatik Ağrı Modelleri
Özet
Nöropatik ağrı, somatosensoriyel sinir sisteminin hasar görmesi veya işlev bozukluğundan kaynaklanan bir kronik ağrı durumudur. Kronik ağrının küresel nüfusun %20'si kadarını etkilediği bilinirken, %10'u kadarının da nöropatik ağrı yaşadığı belirtilmiştir. Mevcut tedavi stratejileri genellikle temel nedenleri ele almak yerine yalnızca semptomatik rahatlama sunmaktadır. Nöropatik ağrının karmaşık mekanizmaları ve mevcut tedavilerin sınırlı etkinliği nedeniyle, altta yatan patofizyolojiyi çözmek ve yeni tedavi stratejileri geliştirmek için sağlam deneysel modeller çok önemlidir. Başlıca kemirgenlerde olmak üzere çeşitli deneysel modellerin geliştirilmesi, periferik sinir yaralanmalarından merkezi sinir sistemi lezyonlarına ve hatta metabolik veya kemoterapi kaynaklı nöropatilere kadar uzanan nöropatik ağrının nörobiyolojik temellerinin incelenmesinde kritik öneme sahiptir. Bu modellerin nöroinflamasyon, anormal plastisite ve değişmiş iyon kanalı işlevi dahil olmak üzere nöropatik ağrıya katkıda bulunan hücresel ve moleküler mekanizmaların açıklığa kavuşturulmasında yararlı oldukları ortaya koyulmaktadır. Bu bölümde, nöropatik ağrının etiyoloji, epidemiyoloji ve patofizyolojisi açıklanarak, deneysel hayvan modelleri üzerinde durulmuştur. Nöropatik ağrının deneysel modellerinin birbirine göre üstünlükleri, dezavantajları açıklanarak uygulama aşamaları belirtilmiştir. Böylece araştırmacılara, çalışmak istedikleri konuya göre en uygun deneysel model seçimi ve yorumlaması konusunda yol gösterici bir çerçeve sunulmaktadır.
Neuropathic pain is a chronic pain condition resulting from damage or dysfunction of the somatosensory nervous system. While chronic pain is known to affect up to 20% of the global population, it is estimated that approximately 10% experience neuropathic pain. Current treatment strategies often offer only symptomatic relief rather than addressing the underlying causes. Due to the complex mechanisms of neuropathic pain and the limited efficacy of current treatments, robust experimental models are crucial for elucidating the underlying pathophysiology and developing novel treatment strategies. The development of various experimental models, primarily in rodents, is critical in investigating the neurobiological basis of neuropathic pain, ranging from peripheral nerve injuries to central nervous system lesions and even metabolic or chemotherapy-induced neuropathies. These models are proving useful in elucidating the cellular and molecular mechanisms contributing to neuropathic pain, including neuroinflammation, abnormal plasticity, and altered ion channel function. This section describes the etiology, epidemiology, and pathophysiology of neuropathic pain, focusing on experimental animal models. The advantages and disadvantages of these experimental models are explained, along with their application stages. This provides researchers with a framework to guide them in selecting and interpreting the most appropriate experimental model for their research topic.
