Acil Serviste Kot Kırıkları ve Flail Chest Tanılı Hastaların Yönetimi
Özet
Kosta kırıkları ve flail chest, acil serviste sık karşılaşılan ve yüksek mortalite ile morbidite riski taşıyan önemli toraks travmaları arasında yer almaktadır. Bu yaralanmalar çoğunlukla motorlu taşıt kazaları ve düşmeler gibi yüksek enerjili künt travmalar sonucunda ortaya çıkmakta, sıklıkla akciğer kontüzyonu, pnömotoraks ve hemotoraks gibi ciddi intratorasik yaralanmalarla birlikte seyretmektedir. Kosta kırıklarında temel patofizyolojik sorun, ağrıya bağlı yetersiz ventilasyon ve sekresyon retansiyonu iken; flail chest, toraks duvarı stabilitesinin bozulması ve paradoksal solunum hareketleri ile karakterizedir. Tanı sürecinde klinik değerlendirme ile birlikte bilgisayarlı tomografi altın standart görüntüleme yöntemidir. Tedavide etkin analjezi, solunum fizyoterapisi ve uygun solunum desteği temel yaklaşımlardır. Seçilmiş olgularda cerrahi kosta stabilizasyonu, mekanik ventilasyon süresini ve komplikasyon oranlarını azaltarak hasta prognozunu iyileştirmektedir. Multidisipliner ve erken müdahaleye dayalı yönetim, klinik sonuçların iyileştirilmesinde kritik öneme sahiptir.
