Depresyon ve Kanıta Dayalı Tedavi Bakım Uygulamaları
Özet
Kronik böbrek yetmezliği yaygın görülen toplum sağlığı sorunlarındandır. Kronik böbrek yetmezliği tanılı bireyler hastalığın getirmiş olduğu birçok fiziksel ve ruhsal semptom yükünü deneyimlemektedirler. Bu sorunlar arasında depresyon ön plana çıkan ve sık görülen ruhsal bozukluklardan birisidir. Depresyonun erken dönemde rutin uygulamalarda tanılanması önemlidir. Bu konuda geliştirilmiş öz bildirime dayalı ölçekler ve klinik yapılandırılmış görüşmeler tanının erken dönemde saptanmasına katkı sağlayabilir. Depresyon tedavisinde farmakolojik ve nonfarmakolojik tedavi yaklaşımları kullanılmaktadır. Farmakolojik tedavi yaklaşımları arasında daha çok SSRI grubu antidepresanlar tercih edilirken, nonfarmakolojik yaklaşımlar arasında Bilişsel Davranışçı Terapi etkililiği açısından öne çıkan psikososyal yaklaşımlardandır. Hemşirelik sürecinin yönetiminde hem fiziksel hem de psikososyal açıdan hastayı bütüncül yaklaşımla ele almak gerekmektedir. Konsültasyon liyezon psikiyatrisi (KLP) hemşirelerinin tanının alınması ile birlikte hastaların ruhsal durumlarını kapsamlı değerlendirmesi ve uygun hemşirelik müdahalelerini planlaması bakımın önemli bir parçasıdır.
