Safra Taşı İleusu: Klinik Olgu Sunumu ve Literatür İncelemesi
Özet
Safra taşı ileusu, safra kesesi ile gastrointestinal sistem arasında gelişen fistül yoluyla büyük boyutlu taşların barsak lümenine geçmesi sonucu ortaya çıkan nadir görülen ciddi bir klinik tablodur. Çoğunlukla yaşlı kadın hastalarda görülmesine rağmen, farklı yaş ve cinsiyet gruplarında da gözlenebilir. Bu olgu sunumunda, 42 yaşında erkek hastada gelişen safra taşı ileusu vakası detaylı olarak incelenmiştir. Hasta karın ağrısı, konstipasyon, bulantı ve kusma şikayetleri ile başvurmuş; yapılan görüntüleme sonuçlarında safra kesesi-duodenum fistülü, jejunum içerisinde taş ile uyumlu görünüm ve taşa bağlı ileus saptanmıştır. Acil cerrahi eksplorasyon sonucunda enterotomi yapılarak taş çıkarılmış, enterotomi primer olarak onarılmıştır. Postoperatif süreçte hasta stabil seyretmiş, hastada herhangi bir komplikasyon gelişmemiş ve hasta şifa ile taburcu edilmiştir. Bu olgu, erken tanı ve uygun cerrahi yaklaşımın safra taşı ileusuna bağlı morbidite ve mortaliteyi azaltmada kritik öneme sahip olduğunu vurgulamaktadır.
