Ağrılı Kraniyal Nöropatilerin Tedavisi
Özet
Bu bölümde, kraniyal sinirlerin tutulmasına bağlı gelişen ve baş–yüz bölgesinde şiddetli ağrıya yol açan ağrılı kraniyal nöropatiler ele alınmaktadır. En sık görülen formlar trigeminal, glossofaringeal ve oksipital nevraljilerdir. Patofizyolojide en yaygın neden vasküler kompresyon olup; demiyelinizan hastalıklar, tümörler, enfeksiyonlar ve travmalar da etiyolojide yer alabilir. Tanı; ayrıntılı klinik değerlendirme, nörolojik muayene ve manyetik rezonans görüntüleme yöntemleriyle konur. Tedavide ilk basamak farmakolojik yaklaşımlar olup, özellikle antiepileptik ilaçlar ön plandadır. Dirençli olgularda girişimsel yöntemler ve cerrahi tedaviler etkili seçenekler sunar. Bireyselleştirilmiş ve multidisipliner yaklaşım, uzun dönem başarı için temel unsurdur.
