Özet

Kalp nakli, ileri evre kalp yetmezliği olan hastalarda yerleşik, etkin ve standart bir tedavi seçeneğidir. Yenilikçi deneysel girişimlerden uzun dönemli klinik başarıya uzanan bu süreç; cerrahi disiplin, organizasyonel gelişmeler ve multidisipliner iş birliğinin bir ürünüdür. Bu sayede cerrahi teknikler, organ koruma yöntemleri, immünsupresyon stratejileri, organ bağışı ve dağıtım sistemleri, enfeksiyon kontrolü ve uzun dönem greft izlemi gibi birçok alanda kayda değer ilerlemeler sağlanmıştır.
Geleneksel olarak “standart kriterler” optimal donör kalbin tanımlanmasında temel alınmıştır. Ancak, nakil listesine alınan hasta sayısının dünya genelinde hızla artması nedeniyle, donör havuzunu genişletmek amacıyla genişletilmiş kriterli donörlerin kullanımı giderek yaygınlaşmaktadır. Bununla birlikte, Türkiye gibi bazı ülkelerde güçlü bir sağlık altyapısına rağmen donör yetersizliği ve aile onayı oranlarının düşüklüğü önemli engeller olmaya devam etmektedir.
Kardiyak donör seçimi ve bakımı, nakil başarısını doğrudan belirleyen en kritik basamaklardan biridir. Beyin ölümü, kalp de dahil olmak üzere birçok organda kompleks patofizyolojik değişikliklere yol açar. Bu süreçlerin anlaşılması ve uygun yönetim stratejilerinin uygulanması, donör organ fonksiyonunun korunması ve transplantasyon sonuçlarının iyileştirilmesi açısından yaşamsal öneme sahiptir.
Donör değerlendirmesinde immünolojik uyum, yaş, cinsiyet, sol ventrikül fonksiyonu, hemodinamik parametreler, biyomarker düzeyleri (troponin, BNP/NT-proBNP), komorbiditeler, koroner arter hastalığı ve enfeksiyon varlığı dikkate alınmalıdır. Ekokardiyografi ve gerekli durumlarda koroner anjiyografi, donör kalbin uygunluğunu belirlemede temel yöntemlerdir. Özellikle koroner arter hastalığı riski yüksek donörlerde anjiyografik değerlendirme önerilmektedir. Ayrıca, donör–alıcı boyut eşleştirmesinde tahmini kalp kütlesi yönteminin kullanılması ve beyin ölümü sonrası gelişen kardiyak disfonksiyonun yoğun bakım tedavisi ile düzeltilebileceği bildirilmektedir.
Donör bakımında ventilatör stratejileri, normoterminin korunması, sıvı-elektrolit dengesinin sağlanması, enfeksiyon profilaksisi, hormonal destek tedavileri ve hiperoksiden kaçınılması kritik öneme sahiptir. Uygun alıcı seçimi ile marjinal donörlerin güvenle kullanılabileceğine dair kanıtlar giderek artmaktadır.
Kalp nakli alanı; organ koruma teknolojilerindeki ilerlemeler, lojistik optimizasyon, soğuk iskemi süresinin azaltılması, mitokondriyal transplantasyon, dolaşım durması sonrası donör kullanımının artışı ve organ dağıtım politikalarındaki güncellemeler ile sürekli gelişmektedir. Bu nedenle, donör seçimi stratejilerinin ve risk öngörü modellerinin dinamik biçimde güncellenmesi, nakil sonuçlarının daha da iyileştirilmesi açısından kritik öneme sahiptir.
