Acil Serviste Yatak Başı Ultrason (Point-Of Care Ultrasound - Pocus) Kullanımı

Özet

Amaç: Bu çalışma, acil serviste yatak başı ultrason (Point-of-Care Ultrasound - POCUS) kullanımının klinik uygulamalardaki yerini, tanısal doğruluğunu, avantajlarını, sınırlılıklarını, yaygın kullanımının önündeki engelleri ve bu engellerin aşılmasına yönelik stratejileri değerlendirmeyi amaçlamaktadır.

Yöntem: Acil serviste POCUS kullanımına ilişkin mevcut literatür kapsamlı bir şekilde taranmış, çeşitli klinik durumlarda POCUS'un tanısal performansını değerlendiren çalışmalar incelenmiştir.

Bulgular: POCUS'un çeşitli klinik durumlar için yüksek tanısal doğruluğa sahip olduğu gösterilmiştir. Apendisit için orta düzeyde duyarlılık ve yüksek özgüllük, hidronefroz için yüksek duyarlılık ve orta düzeyde özgüllük, retina dekolmanı için %94 duyarlılık ve %91 özgüllük, deri ve yumuşak doku apselerinde %95 duyarlılık ve %85 özgüllük değerleri saptanmıştır. Plevral efüzyon tanısında ve akut kalp yetmezliğinde yüksek tanısal performans göstermiştir. Sepsis yönetiminde tanı koyma becerisini artırmış ve hızlı tanı sağlamıştır.

Sonuç: POCUS, acil serviste hızlı, doğru ve radyasyonsuz tanı imkanı sunan değerli bir araçtır. Ancak ekipman eksikliği, yetersiz eğitim, finansman sorunlar gibi önemli sınırlılıklar bulunmaktadır. Kapsamlı eğitim programları, standardizasyon, altyapı iyileştirmeleri ve politika desteği gerekmektedir.
