Acil Serviste Vasküler Erişim Yolları: Periferik İntravenöz ve İntraosseöz Kanülasyon
Özet
Periferik intravenöz kanül; sağlık hizmetlerinde düşük maliyetli, erişimi kolay ve eğitim gereksinimi az bir vasküler erişim yöntemi olsa da, kardiyopulmoner arrest gibi periferik damarların kollabe olduğu durumlarda başarısızlık görülebilir. İntraosseöz kanül ise bu kritik tabloda ilk denemede daha yüksek başarı oranı ve daha hızlı ilaç-sıvı uygulama süresi sunarak güvenilir bir alternatif oluşturmaktadır ve güncel kılavuzlarca da periferik intravenöz kanüle alternatif olarak önerilmektedir. Her iki yöntemin endikasyonları, teknik uygulama özellikleri ve komplikasyon profilleri güncel literatür verileriyle karşılaştırıldığında, vasküler erişim yolu seçimi hastanın klinik durumu, aciliyet düzeyi ve ekipman erişilebilirliği gibi faktörler göz önünde bulundurularak dinamik biçimde değerlendirilmelidir.
