Böbreğin Germ Hücreli Tümörleri
Özet
Böbreğin primer germ hücreli tümörleri, ekstragonadal yerleşimli germ hücreli neoplazmlar arasında son derece nadir görülen, heterojen bir tümör grubunu oluşturur. En sık görülen alt tip matür veya immatür teratom olup, daha nadiren yolk kesesi tümörü (YST) veya mikst teratom–YST şeklinde görülür. Morfolojik olarak gonadal germ hücreli tümörlere benzemekle birlikte, biyolojik olarak farklı bir antite oldukları düşünülmektedir. En yaygın kabul gören hipoteze göre, embriyogenez sırasında göç eden primordiyal germ hücrelerinin nefrojenik dokuda hapsolması sonucu gelişirler. Alternatif bir görüş ise, bu lezyonların regrese olmuş gonadal tümörlerin metastatik odaklarını (“burned-out” tümörler) yansıttığını ileri sürer. Bugüne kadar 25’ten az vaka bildirilmiştir. Olguların çoğu erkeklerde, bebeklikten yetişkinliğe kadar geniş bir yaş aralığında görülür ve bazıları at nalı böbrek veya renal displazi gibi konjenital anomalilerle ilişkilidir. Klinik olarak karın ağrısı, kitle ve serum tümör belirteçlerinde (AFP, β-hCG, LDH) artış görülebilir. Histolojik olarak teratomlar, ektodermal, endodermal ve mezenkimal dokuları içerirken, YST’ler Schiller–Duval cisimcikleri ve hiyalen globüller ile karakterizedir. OCT4, SALL4, PLAP, AFP, β-hCG ve Glipikan-3 immün belirteçleri tanıda yararlıdır. Prognoz histolojik tipe bağlıdır: matür teratomlar benign seyrederken, YST veya immatür komponent içeren olgular daha belirsiz veya agresif davranış gösterebilir.
