İnsan Mikrobiyomu: Sağlık ve Hastalık Üzerindeki Etkileri
Özet
İnsan vücudu, yalnızca kendi hücrelerimizden değil, aynı zamanda trilyonlarca mikroorganizmadan oluşan karmaşık bir ekosistemden meydana gelir. Başta bakteriler olmak üzere arke, mantar, virüs ve protistleri içeren bu topluluk “insan mikrobiyomu” olarak adlandırılır. Bu mikroorganizmalar özellikle bağırsaklarda yoğunlaşmakla birlikte, deri, ağız, solunum yolları ve ürogenital sistem gibi pek çok farklı bölgede de bulunur. İnsan mikrobiyomu, konakçıyla simbiyotik bir ilişki içinde olup sağlığın sürdürülmesinde kritik roller üstlenir. Besin sindirimi, vitamin sentezi, bağışıklık düzenlenmesi, patojenlere karşı koruma ve beyin fonksiyonlarını etkileyen metabolit üretimi bu işlevler arasındadır. Mikrobiyomun yapısında ve işlevinde meydana gelen dengesizlikler yani “disbiyozis” başta inflamatuar bağırsak hastalıkları, obezite, tip 2 diyabet, alerjiler, otoimmün hastalıklar, bazı nörolojik bozukluklar ve çeşitli kanser türleri olmak üzere birçok kronik hastalıkla ilişkilendirilmiştir. Mikrobiyomun sağlık ve hastalık üzerindeki derin etkilerinin anlaşılması, probiyotikler, prebiyotikler ve fekal mikrobiyota nakli (FMT) gibi mikrobiyom hedefli yeni terapötik stratejilerin geliştirilmesine zemin hazırlamaktadır. Bu bölüm, insan mikrobiyomunun tanımını, sağlık üzerindeki kritik rollerini, disbiyozisin hastalıklarla bağlantısını, mikrobiyomu etkileyen faktörleri ve gelecekteki terapötik potansiyelini akademik bir bakış açısıyla ayrıntılı biçimde ele almaktadır.
