Çocuklarda Ağız Solunumu ve Dentofasiyal Etkileri

Özet

Ağız solunumu, çocukluk çağında sık görülen, nazal hava yolunun obstrüksiyonu veya alışkanlık temelli nedenlerle gelişebilen bir solunum paternidir. Uzun süreli ağız solunumu; tükürük akışının işlevlerini zayıflatarak çürük ve periodontal hastalık riskini artırır, halitozise zemin hazırlar ve orofasiyal kas dengesini bozarak dentofasiyal büyüme-gelişimi olumsuz etkiler. Klinik olarak dar “V” tipi maksiller ark, posterior çapraz kapanış, artmış mandibular düzlem açısı, alt anterior yüz yüksekliğinde artış ve Sınıf II/III iskeletsel paternler görülebilir. Etiyolojide adenotonsiller hipertrofi, septum deviasyonu, konka hipertrofisi ve alerjik rinit öne çıkar. Tanıda öykü-alışkanlık sorgulaması, klinik gözlem ve basit testlerin yanı sıra nazal endoskopi ve (uygunsa) polisomnografi değerlidir. Tedavide amaç fonksiyonel dengeyi yeniden kurmaktır: aile temelli alışkanlık yönetimi, oral miyofonksiyonel terapi, ortodontik yaklaşımlar (ör. hızlı üst çene genişletme, fonksiyonel apareyler) ve gerektiğinde cerrahi (adeno/tonsillektomi) birlikte planlanmalıdır. Multidisipliner yönetim (KBB, pediatri, pedodonti, ortodonti, konuşma terapisi) erken dönemde başlatıldığında kraniyofasiyal morfolojiyi ve yaşam kalitesini anlamlı biçimde iyileştirir. Erken tarama, düzenli kontroller ve bireyselleştirilmiş müdahaleler, uzun dönemli kalıcı sonuçlar için esastır.
