Non-İmmün Hidrops Fetalis:Tanı, Etyoloji ve Prenatal Yönetim
Özet
Non-immün hidrops fetalis (NIHF), fetal yumuşak dokularda ve seröz boşluklarda anormal sıvı birikimi ile karakterize, yüksek perinatal mortalite ve morbidite ile ilişkili heterojen bir klinik tablodur. Etiyolojik spektrum oldukça geniştir; kardiyak anomaliler, kromozomal bozukluklar, hematolojik hastalıklar, intrauterin enfeksiyonlar, metabolik bozukluklar ve yapısal malformasyonlar başlıca nedenler arasında yer alır. Tanı genellikle rutin obstetrik ultrasonografi sırasında, en az iki ayrı fetal kompartmanda sıvı birikiminin gösterilmesiyle konur. Prenatal değerlendirmede ayrıntılı ultrasonografi, Doppler incelemeleri, genetik analizler ve enfeksiyon taramaları kritik öneme sahiptir. NIHF’nin ayırıcı tanısı multidisipliner bir yaklaşım gerektirir. Tedavi stratejileri, altta yatan nedene göre değişkenlik gösterir ve intrauterin transfüzyon, maternal antiaritmik tedavi veya torakoamniyotik şant gibi girişimleri içerebilir. Doğum zamanı ve şekli, fetal durumun stabilitesi ve gestasyonel yaş dikkate alınarak belirlenir. Prognoz; etiyoloji, gebelik haftası, hidropsun şiddeti ve uygulanan yönetim stratejilerine bağlı olarak farklılık gösterir. Multidisipliner izlem ve uygun prenatal müdahaleler sağkalımı artırmada temel rol oynar.
