Gebe Hastada Perkütan Girişim Stratejileri
Özet
Gebelikte akut koroner sendrom (AKS), nadir görülmekle birlikte hem maternal hem de fetal mortalite ve morbidite açısından ciddi riskler taşıyan, yaşamı tehdit eden bir klinik tablodur. Artan hemodinamik yük, hiperkoagülabilite ve hormonal değişiklikler patogenezde önemli rol oynarken; aterosklerotik hastalık, spontan koroner arter diseksiyonu, tromboemboli ve koroner vazospazm başlıca etiyolojik nedenlerdir. Erken tanı, risk sınıflandırması ve multidisipliner yaklaşım tedavi başarısında kritik öneme sahiptir. Özellikle ST segment elevasyonlu miyokard enfarktüsü olgularında perkütan koroner girişim (PKG), uygun radyasyon koruma önlemleri, dikkatli farmakolojik tedavi ve fetal izlem ile uygulandığında sağkalımı artıran temel tedavi yöntemidir.
