Fetal Manyetik Rezonans Görüntülemede Bilinmesi Gerekenler
Özet
Fetal Manyetik Rezonans Görüntüleme (MRG), prenatal tanıda ultrasonografinin sınırlı kaldığı durumlarda önemli bir tamamlayıcı yöntemdir. Özellikle maternal obezite, oligohidroamnios, polihidroamnios veyaplasentanın anterior yerleşimi gibi koşullarda MRG, fetal anomalilerin belirlenmesinde değerli katkılar sunar. MRG’nin en sık kullanıldığı alan fetal nörokranium patolojileridir; posterior fossa, kranial orta hat ve sulkasyon anomalilerinde tanısal üstünlük sağlar. Ayrıca fetal omurga, yüz, toraks, batın, ürogenital sistem ve iskelet yapılarının ayrıntılı değerlendirilmesinde de kullanılmaktadır. Fetal MRG’nin standartlaştırılmış raporlaması; gebelik yaşı, amniyotik sıvı miktarı, fetal pozisyon, görüntü kalitesi ve önceki görüntülerle karşılaştırmalı değerlendirme gibi unsurları içermelidir. Bunun yanında maternal bulguların kaydedilmesi de önemlidir. Literatürde fetal MRG’nin USG’ye ek tanısal katkısının beyin anomalilerinde %16’ya kadar ulaştığı bildirilmiştir. Doğru endikasyon ve yapılandırılmış raporlama, yanlış tanı riskini azaltarak hem klinisyen hem de hasta açısından daha güvenilir bilgi sunar. Sonuç olarak fetal MRG, USG’nin yetersiz kaldığı durumlarda tanıyı güçlendiren, multidisipliner yaklaşıma katkı sağlayan bir yöntemdir.
