Restoratif Diş Hekimliğinde Derin Marjin Elevasyonu: Anlamak ve Uygulamak

Özet

Derin Marjin Elevasyonu (DME), subgingival kavite kenarlarını koronal yönde yeniden konumlandırarak izolasyonu ve restoratif işlemleri kolaylaştırmaktadır. Bu teknik, geleneksel olarak uygulanmakta olan kron boyu uzatma ve ortodontik ekstrüzyona daha az invaziv bir alternatif oluşturmaktadır. DME, biyolojik genişliğe zarar vermeden çalışma alanı oluşturmaya, ölçü alımını ve simantasyonu basitleştirmeye yardımcı olmaktadır. Anında dentin örtüleme (IDS) ile birlikte kullanıldığında, adeziv bağlanma dayanıklılığı artmakta ve marjinal sızdırmazlık iyileşmektedir. CAD/CAM indirekt restorasyonlarda ölçü ve yerleştirme sürecini kolaylaştırmakta, periodontal dokulara uyumlu sonuçlar elde edilmesine katkı sağlamaktadır. Bununla birlikte, DME’nin başarısı tam izolasyon sağlanmasına, doğru matris kullanımına ve operatör deneyimine bağlı olmaktadır. Minimal invaziv yaklaşımla diş dokusunu korurken periodontal sağlığı desteklemekte, klinik olarak uzun dönem başarısının daha fazla araştırmaya ihtiyaç duyduğu görülmektedir. Bu bölüm, DME’nin kavramsal temellerini, klinik uygulama ilkelerini ve avantaj-dezavantajlarını kapsamlı biçimde açıklamaktadır.
