İmplant Destekli Kronlarda Malzeme Seçimi ve Oklüzyon
Özet
İmplantların doğal dişlerden farklı olarak periodontal ligamente sahip olmaması, oklüzal kuvvetlerin doğrudan kemiğe iletilmesine neden olur. Bu durum, aşırı yüklenme ve kemik kaybı gibi riskleri beraberinde getirdiğinden, implant koruyucu oklüzyon yaklaşımıyla kuvvetlerin kontrollü ve eksenel biçimde dağıtılması gerekir. Malzeme seçiminde, hem estetik beklentiler hem de mekanik dayanıklılık dikkate alınmalıdır. Arka bölgelerde yüksek çiğneme kuvvetlerine dayanıklı monolitik zirkonya, ön bölgelerde ise doğal dişe en yakın görünümü sağlayan lityum disilikat tercih edilir. Metal-seramik sistemler ise uzun dönem güvenilirliğiyle öne çıkar. Retansiyon yöntemi seçimi de tedavinin başarısını etkiler. Vida tutuculu kronlar kolay bakım ve düşük biyolojik risk sunarken, siman tutuculu kronlar estetik avantaj sağlar. Ti-base kullanımı her iki yöntemde de stabiliteyi artırır. Parafonksiyon (örneğin bruksizm) durumunda monolitik zirkonya tercih edilmeli, gece plağı kullanımıyla koruyucu önlemler alınmalıdır. Dijital sistemler (CAD-CAM, T-Scan) sayesinde oklüzyonun hassas planlanması mümkündür. Sonuç olarak, uygun malzeme, doğru oklüzyon ve dikkatli planlama, implant destekli kronların uzun ömürlü başarısının anahtarıdır.
