Geriatrik Üroonkoloji Cerrahisi Peroperatif Bakım

Özet

Yaşlı nüfusun artışıyla birlikte üroonkolojik cerrahilerde geriatrik hasta yönetimi giderek daha önemli hale gelmektedir. Bu hasta grubunda komorbid hastalıklar, polifarmasi, fonksiyonel ve bilişsel kayıplar cerrahi riskleri artırır. Bu nedenle kapsamlı geriatrik değerlendirme, frajilite analizi ve risk skorlamaları (CCI, ASA, ECOG) cerrahi öncesi mutlaka uygulanmalıdır. Anestezi yönetimi, yaşlanmanın kardiyovasküler, pulmoner ve renal sistem üzerindeki etkileri dikkate alınarak planlanmalıdır. İntraoperatif dönemde doğru pozisyonlama, doku koruması ve minimal invaziv teknikler komplikasyonları azaltabilir. Postoperatif süreçte erken mobilizasyon, pulmoner egzersizler, ERAS protokolleri ve multidisipliner takip komplikasyonları önlemede etkilidir. Geriatrik cerrahilerde amaç yalnızca sağkalımı artırmak değil, aynı zamanda yaşam kalitesini korumaktır. Bu nedenle multidisipliner yaklaşım ve bireyselleştirilmiş tedavi planlaması, güvenli ve etkin sonuçlar elde etmek için kritik önemdedir.
