Gestasyonel Trofoblastik Neoplazi
Özet
Gestasyonel trofoblastik neoplazi (GTN), gebeliğe bağlı bir tümördür ve invaziv mol, koryokarsinom (KKA), plasental bölge trofoblastik tümörü ve epiteloid trofoblastik tümörü (PSTT/ETT) içerir. Gestasyonel trofoblastik hastalıkların en önemli özellikleri hCG üretebilmeleridir. Malignite tanısı konduktan sonra hastalık evrelemesi ve klinik risk skorlaması yapılır. Risk skoruna göre tedavi planlanır.
PSTT/ETT'nin yönetimi farklılık gösterir ve kür oranları biraz daha düşük olma eğilimindedir. Hangi formda olurlarsa olsunlar, gestasyonel trofoblastik neoplazilerde, hCG düzeylerini içerecek uygun değerlendirme ve tedavi ile kür sağlanabilir. Kür sonrası yakın takip programları ile hastalığın olası nüksü erken saptanıp mortalite azaltılabilir.
Gestational trophoblastic neoplasia (GTN) is a pregnancy-related tumour and includes invasive mole, choriocarcinoma (CCA), placental site trophoblastic tumour and epithelioid trophoblastic tumour (PSTT/ETT). The most important feature of gestational trophoblastic disease is the ability to produce hCG. Once a malignancy has been diagnosed, disease staging and clinical risk assessment are carried out. Treatment is planned according to the risk score.
Management of PSTT/ETT differs and cure rates tend to be slightly lower. Regardless of the form of gestational trophoblastic neoplasia, cure can be achieved with appropriate assessment and treatment, including hCG levels. Close follow-up after cure can detect recurrence early and reduce mortality.
