Dental İmplantasyonda Osteointegrasyonu Etkileyen Faktörler ve Stabilite Değerlendirme Yöntemleri

Bu bölüm, dental implantasyon sürecinde osseointegrasyonu etkileyen faktörleri ve implant stabilitesini değerlendirme yöntemlerini ele almaktadır. Osseointegrasyon, implant yüzeyi ile kemik dokusu arasında fonksiyonel ve yapısal bağlantının oluştuğu biyolojik bir süreçtir. Bu sürecin başarısı, biyomekanik stabilite, kemik yoğunluğu, cerrahi teknik, implant yüzey özellikleri ve bireysel biyolojik faktörler gibi çeşitli değişkenlere bağlıdır. İmplant stabilitesi, primer ve sekonder stabilite olmak üzere iki aşamada değerlendirilir. Primer stabilite, implantın yerleştirme anındaki mekanik tutunmasını ifade ederken, sekonder stabilite kemik dokusunun yeniden şekillenmesiyle elde edilen biyolojik entegrasyonu kapsar. Başarılı bir osseointegrasyon süreci, primer stabilitenin korunması ve sekonder stabilitenin zamanla güçlenmesiyle mümkündür. Bu bağlamda, implant stabilitesinin değerlendirilmesi amacıyla histolojik analizler, radyografik incelemeler, yerleştirme torku ölçümleri, periotest uygulamaları ve rezonans frekans analizi (RFA) gibi çeşitli yöntemler kullanılmaktadır. Bu yöntemlerin klinik uygulanabilirliği, doğruluk ve güvenilirlik açısından karşılaştırılarak ele alınmıştır. Sonuç olarak, osseointegrasyonu destekleyen faktörlerin anlaşılması ve uygun değerlendirme yöntemlerinin seçilmesi, dental implantların uzun vadeli başarısını artırmada kritik bir rol oynamaktadır.

This chapter examines the factors influencing osseointegration in the dental implantation process and the methods used to evaluate implant stability. Osseointegration is a biological process in which a functional and structural connection is established between the implant surface and the bone tissue. The success of this process depends on various factors, including biomechanical stability, bone density, surgical technique, implant surface characteristics, and individual biological variables. Implant stability is assessed in two stages: primary and secondary stability. Primary stability refers to the mechanical retention of the implant at the time of placement, while secondary stability involves the biological integration achieved through bone remodeling. A successful osseointegration process requires maintaining primary stability and progressively strengthening secondary stability over time. In this context, various methods such as histological analysis, radiographic examinations, insertion torque measurements, Periotest applications, and resonance frequency analysis (RFA) are utilized to assess implant stability. These methods are analyzed in terms of their clinical applicability, accuracy, and reliability. In conclusion, understanding the factors that support osseointegration and selecting appropriate assessment methods play a critical role in enhancing the long-term success of dental implants.
