Endodontik Tedavide Ağrının Önemi ve Postoperatif Ağrı Kontrolünde Uygulanabilecek Yöntemler
Özet
Ağrı, doku hasarıyla ilişkili hoş olmayan bir duyusal ve duygusal deneyim olarak tanımlanmaktadır. Nörofizyolojik mekanizmalar, nosiseptörler ve sinir lifleri (A-delta ve C lifleri) aracılığıyla ağrı iletimini açıklar. Diş ağrısı, dentin lenfinin hidrodinamik hareketiyle tetiklenir ve periferik/santral sensitizasyon süreçleriyle şiddetlenip hastalarda istenmeyen ağrı olgusuyla sonuçlanabilmektedir. Endodontik tedavi sırasında gelişen ağrı ise, preoperatif , intraoperatif ve postoperatif (tedavi sonrası) evrelerde görülmektedir. Hastalarda preoperatif ağrı varlığı, postoperatif ağrı riskini artırmaktadır. Tedavi sırasında oluşan ağrı ise preoperatif ağrı olarak tanımlanmakta, anestezi yetersizliği, kök kanalından debris taşması ve inflamasyon gibi birçok duruma bağlı olarak gelişebilmektedir. Postoperatif ağrı; hasta faktörleri (yaş, cinsiyet, anksiyete), diş tipi , pulpa durumu ve tedavi teknikleri ile ilişkilidir. Yanlış çalışma boyu yada apikal foramenin aşırı genişletilmesi gibi durumlara bağlı olarak ağrı tetiklenebilmektedir. Postoperatif ağrının kontrol altına alınması için zaman içerisinde farmakolojik ve farmakolojik olmayan bir çok yöntem önerilmiştir. Farmakolojik yöntemler arasında non-steroid antiinflamatuarlar (NSAİİ) (ibuprofen), asetaminofen ve kortikosteroidlerin reçete edilmesi öne çıkmaktayken, non-farmakolojik yaklaşımlara düşük dalga boylu lazer terapisi, kriyoterapi ve ozon uygulaması örnek olarak verilebilir. Her üç tedavi yöntemi de enflamasyonu ve ağrıyı azaltmada etkili olarak bulunmuştur. Bu bölümde ise birçok nedene bağlı oluşabilen postoperatif ağrıyı nedenleriyle beraber incelemek ve kontrol altına alınmasıyla ilgili yapılan araştırmaları derleyerek hekimlere çeşitli önerilerde bulunmak amaçlanmıştır.
Pain is defined as an unpleasant sensory and emotional experience associated with tissue damage. Neurophysiological mechanisms elucidate pain transmission through nociceptors and nerve fibers (A-delta and C fibers). Dental pain is primarily triggered by the hydrodynamic movement of dentinal fluid and can be exacerbated by peripheral and central sensitization processes, potentially leading to persistent pain in patients. During endodontic treatment, pain may manifest at different stages: preoperative, intraoperative, and postoperative (post-treatment). The presence of preoperative pain has been shown to increase the likelihood of postoperative pain. Intraoperative pain is generally classified as preoperative pain and may result from factors such as inadequate anesthesia, extrusion of debris from the root canal, or inflammatory responses. Postoperative pain is influenced by several factors including patient-specific variables (age, gender, anxiety), tooth type, pulp condition, and treatment techniques. Pain can be triggered by procedural errors, such as incorrect working length or excessive enlargement of the apical foramen. Various pharmacological and non-pharmacological strategies have been proposed over time to manage postoperative pain. Pharmacological interventions commonly include non-steroidal anti-inflammatory drugs (NSAIDs, e.g., ibuprofen), acetaminophen, and corticosteroids, while non-pharmacological approaches encompass low-wavelength laser therapy, cryotherapy, and ozone application. All three therapeutic modalities have demonstrated efficacy in reducing both inflammation and pain. This review aims to analyze the etiology of postoperative pain, explore the underlying mechanisms, and provide a comprehensive synthesis of studies focusing on pain management strategies, offering evidence-based recommendations for clinicians.”
