Kök Rezorpsiyonunun Tanı ve Tedavisinde Güncel Yaklaşımlar

Özet

Kök rezorpsiyonu, odontoklastik aktivite nedeniyle diş sert dokularının kaybı olarak tanımlanmaktadır. Primer dentisyonda rezorpsiyon fizyolojik bir süreç olarak kabul edilir ve süt dişlerinin eksfoliasyonuna yardımcı olur. Ancak daimî dentisyonda rezorpsiyon patolojiktir ve istenmeyen bir durumdur. Rezorpsiyon sürecinin başlaması için gerekli olanlar; dokudaki doğal bariyerin yıkılması, sürekli bir stimülasyon ve klastik hücreler için canlı bir kan kaynağı olarak sıralanabilir. Dış doğal bariyerler periodontal ligament ve sement iken iç doğal bariyerler odontoblastlar ve predentindir. Bu bariyerler klastik hücrelerin sement ve dentinin mineralize olmamış matriksine tutunmasını ve rezorbe olmasını önler. Rezorpsiyon için uyarıcı faktörler kök kanal sisteminde bakteri varlığı, travmatik yaralanmalar, gelişimsel kusurlar, komşu gömülü diş, kist veya tümör benzeri patolojiler olabilir.
Kök rezorpsiyonu genellikle asemptomatiktir; bu nedenle radyografik inceleme ile tesadüfen ortaya çıkar. İlerleyen vakalarda ağrı, renklenme ve mobilite gibi klinik bulgular gözlenebilir. Kök rezorpsiyonuna yol açan nedenlerin ve risk faktörlerinin bilinmesi ve düzenli radyografik kontrollerin yapılması, rezorpsiyonun erken evrede teşhis edilmesini ve ilerlemesinin önlenmesini mümkün kılar. Kök rezorpsiyonu başlangıç, rezorpsiyon ve onarım olmak üzere üç aşamada gerçekleşir. Rezorpsiyon, kendi kendini sınırlayabilir ve klinik olarak fark edilmeyebilir. Ancak rezorptif süreç ilerlediğinde diş sert dokularında kayıp meydana gelebilmektedir. Kök rezorpsiyonu uygun bir şekilde teşhis ve tedavi edilmediğinde diş sert dokularında kayıp ilerlemekte ve bu durum dişin çekimi ile sonuçlanabilmektedir.

 

Root resorption is defined as the loss of dental hard tissues due to odontoclastic activity. In the primary dentition, resorption is considered a physiological process that facilitates the exfoliation of deciduous teeth. However, in permanent dentition, resorption is pathological and represents an undesirable condition. The initiation of the resorption process requires three key factors: destruction of the natural protective barrier, continuous stimulation, and a viable blood supply for clastic cells. The external natural barriers are the periodontal ligament and cementum, while the internal barriers are odontoblasts and predentin. These barriers prevent clastic cells from adhering to and resorbing the unmineralized matrix of cementum and dentin. Several stimuli may trigger root resorption, including the presence of bacteria in the root canal system, traumatic injuries, developmental anomalies, neighboring impacted teeth, and pathological lesions such as cysts or tumors.
Root resorption is often asymptomatic and usually detected incidentally through radiographic examinations. In advanced cases, clinical signs such as pain, discoloration, and tooth mobility may be observed. Awareness of the causes and risk factors of root resorption, along with regular radiographic monitoring, is essential for early diagnosis and prevention of progression. Root resorption occurs in three phases: initiation, resorption, and repair. The process may be self-limiting and remain clinically undetectable. However, as resorption progresses, significant loss of dental hard tissues can occur. If not properly diagnosed and treated, root resorption may lead to extensive tissue loss, ultimately resulting in tooth extraction.
