Febril Konvülziyon
Özet
Febril nöbetler çocukluk yaş grubunda en sık görülen nörolojik bozukluktur. Altı ay ile beş yaş arasında daha önce nöbet geçirmeyen çocuklarda, ateş dışında tanımlanabilir bir neden olmaksızın (örneğin santral sinir sisteminin enfeksiyonu, metabolik bozukluk, travma veya zehirlenme gibi) ateşle ilişkilendirilen nöbetler olarak tanımlanır. Febril nöbetler, genellikle benign ve sınırlayıcı bir karakter taşır. Tipik olarak, çocuklar 6 yaşına geldiğinde bu durumdan kurtulurlar. Çoğu ataklar kısadır ve kendiliğinden sonlanır, bu nedenle uzun süreli antiepileptik ilaç tedavisi gerektirmez. AAP, Hib ve Streptococcus pneumoniae için aşı durumu bilinmeyen veya eksik aşılı olan 12 ayın altındaki çocuklarda ateşli nöbetle başvuranlarda lomber ponksiyon şiddetle önermektedir. Febril nöbet geçiren bir çocuğu değerlendirirken kırmızı bayrakların tanınması önemlidir, çünkü bunlar ek tedavinin gerekip gerekmediğini belirlemeye yardımcı olur. Komplike febril nöbetlerle başvurunun olması, meningeal irritasyon bulgularının varlığı kernig ve brudzinski belirtilerinin pozitif olması, ön fontanel şişkinliği kırmızı bayrak belirtileridir.
