Cerrahi Sürecinde Olan Çocuk ve Ergenlerde Konsültasyon-Liyezon Psikiyatrisi
Özet
Bu bölüm, cerrahi süreçten geçen çocuk ve ergenlerde Konsültasyon Liyezon Psikiyatrisi uygulamalarını ele alıyor.
Özetle: Cerrahi işlemler, çocuk ve aileler için stresli bir deneyim olabilir. Çocukların yaşlarına ve bilişsel gelişim seviyelerine uygun destek sağlanmalıdır. 0-2 yaş grubu çocuklar bakım verenlerine bağımlıyken, 2-7 yaş grubunda büyüsel düşünme yaygındır. 7-11 yaş döneminde ise mantıksal düşünce gelişirken, ergenler ise bağımsızlık kaybı ve estetik kaygılar yaşayabilir. Nörogelişimsel bozukluğu olan çocukların cerrahi süreci daha karmaşıktır ve özel yaklaşımlar gerektirir. Ayrıca, ebeveynlerin kaygıları da çocuğun psikolojik durumunu etkileyebilir. Preoperatif hazırlık, çocukların ve ailelerin bilgilendirilmesini ve kaygının yönetilmesini içerir. Anksiyolitik premedikasyon, çocukların cerrahi öncesi rahatlatılmasını amaçlar. Bazı özel cerrahi durumlar (sünnet, ampütasyon gibi) çocuklarda psikolojik etkiler yaratabilir. Özellikle sünnetin yanlış yaşta yapılması kastrasyon anksiyetesi doğurabilir. Ampütasyon, beden imajı ve psikolojik uyum açısından önemli zorluklar yaratır. Psikiyatrik ilaç tedavisinin cerrahi süreçte nasıl yönetileceği de detaylandırılmıştır. Ayrıca, ağrı yönetimi ve pediatrik deliryum gibi durumların nasıl ele alınması gerektiği açıklanmıştır.
Özetle, çocukların cerrahi süreci psikolojik olarak sağlıklı yönetildiğinde, hem çocuk hem de aile için daha az travmatik hale gelebilir.

In summary: Surgical procedures can be a stressful experience for both children and their families. Support should be tailored to children's age and cognitive development level. Children aged 0-2 are highly dependent on their caregivers, while those aged 2-7 tend to engage in magical thinking. In the 7-11 age group, logical reasoning starts to develop, whereas adolescents may experience concerns related to loss of independence and cosmetic outcomes. Children with neurodevelopmental disorders require a more complex approach during surgical processes. Additionally, parental anxiety can significantly impact a child psychological state. Preoperative preparation involves informing children and their families and managing anxiety. Anxiolytic premedication aims to help children feel more at ease before surgery. Certain specific surgical conditions (such as circumcision and amputation) can have psychological effects on children. Circumcision performed at an inappropriate age may lead to castration anxiety. Amputation can present challenges related to body image and psychological adaptation. The document also details how psychiatric medication should be managed during the surgical process. Furthermore, it explains approaches to pain management and pediatric delirium.
In summary, when the surgical process is managed with psychological support, it can be less traumatic for both the child and their family.
