Minimally Invasive Multi-Valve Surgery

Özet

This chapter discusses the development, techniques, and benefits of minimally invasive multi-valve surgery. Compared to traditional sternotomy, minimally invasive surgery uses smaller incisions, resulting in less trauma. This approach is particularly preferred when interventions on the mitral, tricuspid, and aortic valves are required simultaneously.
Advantages of minimally invasive multi-valve surgery include reduced blood loss, less pain, shorter hospital stays, and better cosmetic outcomes. However, the limited surgical field can make the procedure technically challenging. Preoperative evaluations must carefully assess the patient’s anatomy and any comorbid conditions. Anesthesia management and patient positioning are critical to surgical success.
During the procedure, femoral artery and venous cannulation are commonly used, though central cannulation may be necessary in some cases. Crystalloid cardioplegia is often utilized for myocardial protection. Surgeons may also employ video-assisted techniques during valve repair or replacement. Despite its complexity, minimally invasive multi-valve surgery has demonstrated excellent outcomes even in challenging cases.

Minimal invaziv cerrahi, geleneksel sternotomiye kıyasla daha küçük kesilerle yapılmakta ve daha az travmaya neden olmaktadır. Bu yaklaşım, özellikle mitral, triküspit ve aort kapaklarının aynı anda müdahale edilmesi gereken durumlarda tercih edilmektedir.
Minimal invaziv çoklu kapak cerrahisinin avantajları arasında daha az kan kaybı, daha az ağrı, daha kısa hastanede kalış süresi ve daha iyi kozmetik sonuçlar yer alır. Ancak cerrahi alanın dar olması, işlemi teknik olarak zorlaştırabilir. Preoperatif değerlendirmede hastanın anatomik özellikleri ve eşlik eden hastalıklar dikkatle incelenmelidir. Anestezi yönetimi ve hastanın pozisyonlandırılması cerrahinin başarısı için kritik öneme sahiptir.
Cerrahi sırasında femoral arter ve venöz kanülasyon sıkça tercih edilirken, bazı durumlarda santral kanülasyon da uygulanabilir. Miyokard koruması için genellikle kristalloid kardiopleji kullanılır. Cerrahlar, kapak onarımı veya değişimi sırasında video yardımlı tekniklerden de faydalanabilir. Minimal invaziv çoklu kapak cerrahisi, karmaşık vakalarda bile başarılı sonuçlar verebilmektedir.
