Yoğun Bakımda Kafa Travmalı Hasta Yönetimi

Özet

Trafik kazası nedeniyle 22 yaşında erkek hasta acil serviste değerlendirildi. Glaskow koma skalası 7 (G3V2M2) olması üzerine hasta entübe edildi. Kafa travması nedeniyle çökme fraktürü, kanama ve ödem olan hasta acil dekompresyon cerrahisi sonrası yoğun bakıma getirildi. Hastanın ek olarak pnömotoraks olması nedeniyle acil serviste göğüs cerrahisi tarafından tüp torakostomi ve yatışının 17. gününde ortopedi tarafından tibia fraktürü nedeniyle operasyon yapıldı. Hastanın yoğun bakım geliş sürecinde 48 saat propofol ile sedasyon uygulandı. Hasta 8. gün GKS 15 olması üzerine ekstübe edildi. Enfeksiyona yönelik antibiyoterapisi düzenlendi ve stabil olduktan sonra hasta 30. günde servise devredildi. Burada, travmatik beyin hasarı olan hasta ve yoğun bakım süreci paylaşılacak ve bu hasta grubunda yoğun bakım yönetiminden bahsedilecektir.
