Parainfluenza Virüsü Enfeksiyonları

Özet

Paramiksovirüs ailesinden olan, parainfluenza virüsleri (PIV'ler), erişkinlerde ve çocuklarda solunum yolu enfeksiyonlarının önemli bir nedenidir. Paramyxoviridae ailesindeki Paramyxovirus cinsinde yer alan tek zincirli, zarflı RNA virüsleridir. İnsan PIV'lerinin dört ana serotipi (PIV-1, -2, -3 ve -4) tanımlanmıştır. Parainfluenza virüsler ilk olarak burun ve orofarinks epitelini enfekte eder ve buradan alt solunum yollarına doğru yayılabilir. PIV-1 ve PIV-2 çoğunlukla üst solunum yolu epitelinde çoğalarak üst solunum yolu enfeksiyonlarına ve krupa yol açmaktadır. PIV-3 ise alt solunum yollarında çoğalır; enfekte bronşiyolit ve pnömoniye sebep olabilir. Erişkinlerde en sık pnömoniye yol açan ve hastaneye yatış gerektiren serotip PIV-3’tür, genellikle ilkbahar sonu yaz başında insidansı artmaktadır. PIV genellikle damlacıkların veya fomitlerin solunmasıyla bulaşır. ÜSYE ve akut bronşit en yaygın görülen klinik tablolardır. PIV enfeksiyonunda spesifik mikrobiyolojik tanı hastalığın şiddetine ve hastanın immün durumuna bağlı değişmektedir. Hastaneye yatış gerektiren toplum kökenli pnömonilerde ve solunum yolu enfeksiyonu bulguları olan immünosüprese hastalarda diğer solunum yolu patojenleriyle birlikte PIV’in test edilmesi önerilmektedir. PIV enfeksiyonlarının tedavisi için onaylanmış, etkinliği kanıtlanmış antiviral ilaçlar şu anda mevcut değildir. Semptomlara yönelik destek tedavisi uygulanır. Günümüzde PIV enfeksiyonunu önlemek için lisanslı bir aşı yoktur, ancak aşı geliştirme çalışmaları devam etmektedir.

Parainfluenza viruses (PIVs), which belong to the paramyxovirus family, are significant causes of respiratory tract infections in both adults and children. Parainfluenza viruses initially infect the epithelial cells of the nose and oropharynx, from which they can spread to the lower respiratory tract. PIV-1 and PIV-2 primarily replicate in the upper respiratory tract epithelium, leading to upper respiratory tract infections and croup. In contrast, PIV-3 replicates in the lower respiratory tract and can cause bronchiolitis and pneumonia. The serotype most commonly associated with pneumonia and hospitalizations in adults is PIV-3, which typically shows an increase in incidence from late spring to early summer. Upper respiratory tract infections and acute bronchitis are the most commonly observed clinical presentations. The specific microbiological diagnosis of PIV infection varies depending on the severity of the disease and the patient's immune status. It is recommended to test for PIV along with other respiratory pathogens in cases of community-acquired pneumonia requiring hospitalization and in immunosuppressed patients presenting with respiratory infection symptoms. Currently, there are no approved antiviral medications with proven efficacy for the treatment of PIV infections. Supportive treatment is provided for the symptoms. Currently, there is no licensed vaccine to prevent PIV infections.
