Lomber Sinoviyal Kistin Spontan Gerilemesi: Bir Vaka Sunumu
Özet
Sinovial kistler, genellikle lomber omurga bölgesinde dejeneratif değişikliklerle ortaya çıkan, sinovial sıvı dolu kistik yapılardır. Bel ve bacak ağrısı ile nörolojik semptomlar oluşturabilirler. Bu tür vakalarda cerrahi tedavi sıklıkla önerilse de bazı nadir durumlarda sinovial kistler kendiliğinden gerileyebilir. 71 yaşında kadın hasta, bel ve sağ bacak ağrısı şikâyetleri ile başvurdu. Nörolojik muayenede sağ bacakta hafif güç ve duyu kaybı mevcuttu. Lomber magnetik rözanans görüntülemede L5-S1 seviyesinde sağ S1 sinir köküne bası yapan, 1.5 cm çapında bir sinovial kist görüldü. Hastaya cerrahi müdahale önerilmesine rağmen hasta cerrahiyi reddetti ve konservatif tedavi (nonsteroid antienflamatuar ilaçlar) tercih edildi. Bir yıl sonraki kontrollerde kistin tamamen kaybolduğu ve hastanın semptomlarının düzeldiği tespit edildi. Sinovial kistlerin tedavisinde cerrahi yaygın olarak tercih edilse de bazı vakalarda cerrahi müdahale olmadan kistin küçüldüğü veya kaybolduğu bildirilmiştir. Bu vaka, sinovial kistlerin nadir durumlarda spontan olarak gerileyebileceğini ve konservatif tedaviyle başarılı bir şekilde takip edilebileceğini göstermektedir. Ancak, ciddi nörolojik semptomlar varsa cerrahi tedavi hala önemli bir seçenektir.
Synovial cysts are fluid-filled cystic structures that typically arise from degenerative changes in the lumbar spine. They can cause back and leg pain, as well as neurological symptoms. Although surgical treatment is often recommended in such cases, in some rare instances, synovial cysts may regress spontaneously. A 71-year-old female patient presented with complaints of back and right leg pain. Neurological examination revealed mild motor weakness and sensory loss in the right leg. Lumbar Magnetic Resonance Imaging revealed a 1.5 cm synovial cyst at the L5-S1 level, compressing the right S1 nerve root. Surgical intervention was recommended, but the patient opted for conservative treatment, including nonsteroidal anti-inflammatory drugs and physical therapy.One year later, follow-up imaging demonstrated complete resolution of the cyst, and the patient's symptoms had resolved. While surgery is commonly used to treat synovial cysts, there are cases where the cyst diminishes or disappears without surgery. This case illustrates that synovial cysts can, in rare cases, spontaneously regress, and conservative management may be a viable option. However, in the presence of severe neurological symptoms, surgery remains an important treatment option.
