Terminal Dönemde Hasta Bakımı

Terminal dönem, hastalığın son evresinde bulunan ya da herhangi bir sebeple ölmek üzere olan hastaların hayatlarının son kısmına denilmektedir. Bu dönemde yaşamsal fonksiyonlarının geriye dönüşsüzdür, ölüm sürecine girilmiştir. Terminal dönem hasta bakımında temel amaç, hastanın fiziksel ve psikolojik yönden rahatlığının sağlanması ve kalan yaşam süresinin kalitesinin artırılması olmalıdır. Koşuyolu Yüksek İhtisas Eğitim ve Araştırma Hastanesi Gastroenteroloji Kliniğimizde son evre kanser hastaları da yatmaktadır. Bu hastaların kanser tüm vücutlarına yayıldıkları için çoğunluğu terminal dönem hastası olarak tanımlanabilir. Üç amacımız bulunmaktadır. Bir beslenmeleri, iki enfeksiyondan korunmaları, üç ağrı   duymamalıdır. Terminal dönem hastaların büyük bir kısmı ağrı çekerler. Ağrı kanser süresince sık görülmesine karşın, yaşamın sonuna doğru görülme sıklığı ve şiddeti oldukça artmaktadır. Ağrısı giderilemeyen ya da kontrol altına alınamayan bireylerin günlük yaşam aktivitelerini yerine getirmesi zorlaşır. Terminal dönemde olan hasta ölümden çok ölümün süreci ile baş etmektedir. Hastaların rahat ettirilmesi hastaya bütüncül bakış açısıyla yaklaşılması gerekir. Terminal dönemde olan hastalarımız, bilinçlerini kaybetmiş olsalar dahi etrafındakileri hissedebilir. Bu sebeple hastalarla olan dokunsal ve sözel ilişki kesilmemelidir böylece hasta yalnız olmadığını ve güvende olduğunu hissedecektir. Aynı zamanda bu dönemde hastanın ailesi ve sosyal çevresi ile iş birliği yapılmalı ve hastanın son günlerinde yakınlarıyla yeterince zaman geçirmesine olanak tanınmalıdır.

The main purpose of terminal patient care should be to ensure the physical and psychological comfort of the patient and to increase the quality of the remaining life. End-stage cancer patients are also hospitalized in our hospital's Gastroenterology Clinic. The majority of these patients can be described as terminal stage patients because the cancer has spread throughout their body. We have three purposes. One, they need to be fed, two, they need to be protected from infection, and three, they shouldn't feel any pain. The majority of terminally ill patients experience pain. Although pain is common during cancer, its frequency and severity increases towards the end of life.It becomes difficult for individuals whose pain cannot be relieved or controlled to perform daily living activities. The patient in the terminal period is coping with the process of death rather than death itself. Patients should be made comfortable and the patient should be approached from a holistic perspective. Our terminally ill patients can feel those around them even if they have lost consciousness. For this reason, tactile and verbal relationships with patients should not be interrupted, so that the patient will feel that he is not alone and safe. At the same time, cooperation should be made with the patient's family and social circle during this period, and the patient should be allowed to spend enough time with her relatives in her last days.
