Cerrahi Hastası ve Psikososyal Bakım
Özet
Tıbbi veya cerrahi nedenlerle hospitalize edilen bir bireyde, hastalık ya da hastane ortamıyla ilgili olarak psikolojik tepkilerin ortaya çıktığı ve psikososyal sorunların yaygın olduğu bilinmektedir. Hastada psikososyal sorun varlığı, ameliyat öncesi aşamadan ameliyat sonrası iyileşmeye kadar olan cerrahi süreçte, bağışıklık sistemini, yara iyileşme oranlarını, enfeksiyonlara duyarlılığı ve ameliyat sonrası genel refahı etkilemektedir. Hastaların biyopsikososyal olarak ele alınması ve hemşirelik bakımının bütüncül olması gerekmektedir. Psikososyal bakım, kapsamlı ve kaliteli bakımın önemli bir bileşeni, bütüncül ve hasta merkezli hemşirelik bakımının esas parçasıdır, genel tedavi ve bakımdan ayrılamaz. Ülkemizde yayınlanan hemşirelik yönetmeliğine göre de psikososyal bakım, hemşirelerin görevleri ve yetkisi dahilindedir. Psikososyal bakım, sağlığın kaybıyla ortaya çıkan varoluşsal krizlerle ya da ruhsal sorunlarla baş edemeyen, hastalık sırasındaki duygusal değişimleri düzenleyemeyen bireylere terapötik iletişim yoluyla psikolojik, sosyal ve manevi bakımın kültürel açıdan hassas bir şekilde verilmesidir. İyi bir psikososyal bakım hastaların sağlık sonuçlarını iyileştirmekte ve hastanede kalış süresini azaltmaktadır. Bunun için hemşirelerin cerrahi hastalarının yaşadığı ruhsal sorunları zamanında ve etkili bir şekilde belirleme, soruna yönelik gerekli girişimleri planlama, uygulama ve değerlendirebilme becerisine sahip olması gerekmektedir. Psikososyal bakımın cerrahi servis ortamına entegre edilebilmesi için bu konuda yetkin konsültasyon liyezon psikiyatrisi hemşireleri ekibe rol model olabilir, eğitim ve danışmanlık sunabilir. Sonuçta, psikososyal bakım becerilerinin geliştirilmesi sadece hastaların iyiliklerine katkıda bulunmayacak, aynı zamanda hemşirelerin de daha doyumlu hissetmelerini sağlayacaktır.
It is known that psychological reactions related to the disease, or the hospital environment occur, and psychosocial problems are common in an individual who is hospitalized for medical or surgical reasons. The presence of psychosocial problems in the patient affects the immune system, wound healing rates, susceptibility to infections, and general postoperative well-being during the surgical process from the preoperative stage to postoperative recovery. Patients should be handled biopsychosocial and nursing care should be holistic. Psychosocial care is an important component of comprehensive and quality care, an essential part of holistic and patient-centered nursing care and cannot be separated from general treatment and care. According to the nursing regulations published in our country, psychosocial care is within the duties and authority of nurses. Psychosocial care is the provision of psychological, social, and spiritual care in a culturally sensitive manner through therapeutic communication to individuals who cannot cope with the existential crises or mental problems that arise with the loss of health or who cannot regulate emotional changes during the disease. Good psychosocial care improves patients' health outcomes and reduces the length of hospital stay. For this, nurses must have the ability to identify the psychological problems experienced by surgical patients in a timely and effective manner, and to plan, implement, and evaluate the necessary interventions for the problem. To integrate psychosocial care into the surgical service environment, consultation-liaison psychiatry nurses who are competent in this regard can be role models for the team and provide training and consultancy. Ultimately, improving psychosocial care skills will not only contribute to the well-being of patients but will also make nurses feel more satisfied.
