İdiyopatik Fotosensitif Oksipital Lob Epilepsi (IFOLE)

Özet

Oksipital lob epilepsileri % 5- 10 oranında görülmekte, spesifik semptomları nedeniyle tanınması güç olabilmektedir. Etiyolojik olarak semptomatik ve idiyopatik grupta incelenmektedir. İdiyopatik grup, çocukluk çağı benign parsiyel epilepsileri arasında %20-25 oranında saptanmaktadır. En sık vizüel ve okülomotor iktal semptomlar görülmektedir. Fotosensitivite beynin anormal görsel duyarlılık yanıtı şeklinde tanımlanmaktadır. Sıklıkla ergenlerde ve kadınlarda görülmektedir. Epilepsi hastalarında %5 oranında olduğu bilinmektedir. Genetik olarak kalıtıldığı bilinen fotosensitivite; absans, göz kapağı miyoklonisi, jeneralize / fokal miyoklonik sıçramalar, fokal nöbetler,  jeneralize tonik klonik nöbetler ve tonik versif nöbetlerle ilişkili olduğu bilinmektedir. Tedavide önleyici yöntemler ailelere açıklanmalı ve ataklar engellenmeye çalışılmalı, gerekiyorsa uygun antiepileptik tedavi önerilmelidir.

Occipital lobe epilepsies are seen in 5-10% of cases and may be difficult to recognize due to their specific symptoms. Etiologically, it is studied in the symptomatic and idiopathic group. The idiopathic group is detected at a rate of 20-25% among childhood benign partial epilepsies. The most common visual and oculomotor ictal symptoms are observed. Photosensitivity is defined as an abnormal visual sensitivity response of the brain. It is often seen in adolescents and women. It is known that it occurs in 5% of epilepsy patients. Genetically inherited photosensitivity is known to be associated with abscess, eyelid myoclonia, generalized / focal myoclonic jercs, focal seizures, generalized tonic-clonic seizures and tonic-versive seizures. Preventive methods of treatment should be explained to families and attacks should be tried to be prevented, appropriate antiepileptic treatment should be recommended if necessary.
