Pankreas Nakli ve Hemşirelik Bakımı

Özet

Pankreas nakli, tek başına pankreas nakli, böbrek sonrası pankreas nakli veya eşzamanlı pankreas-böbrek nakli şeklinde yapılabilmektedir. Pankreas nakli öncesinde, kardiyovasküler durum, böbrek fonksiyonları, obezite ve hiperkoagülabilitenin değerlendirilmesi önemlidir. Peritoneal greft yerleştirme, vasküler anastomoz, venöz drenaj ve ekzokrin drenajda çeşitli cerrahi teknikler kullanılmaktadır. Nakil sonrası, greft trombozu, kanama, anastomoz kaçağı, pankreatit ve enfeksiyon en sık rastlanan erken dönem komplikasyonlardır. Nakil sonrası ilk üç ayda kardiyoserebrovasküler olaylar ve enfeksiyonlar mortalitenin ana nedenidir. Hemşirelik bakımında, ameliyat öncesi değerlendirme, ameliyat sırası yönetimde cerrahi ekibin bir üyesi olarak rol alma, ameliyat sonrası yakın izlem ile komplikasyonları erken tanılama temel ilkelerdir.

Pancreas transplantation can be performed as pancreas transplant alone, pancreas after kidney transplant or simultaneous pancreas-kidney transplant. Before pancreas transplantation, it is important to evaluate cardiovascular status, renal functions, obesity and hypercoagulability. Various surgical techniques are used for peritoneal graft placement, vascular anastomosis, venous drainage and exocrine drainage. Graft thrombosis, bleeding, anastomotic leakage, pancreatitis and infection are the most common early post-transplant complications. Cardiocerebrovascular events and infections are the main causes of mortality in the first three months after transplantation. The basic principles of nursing care are preoperative evaluation, taking part as a member of the surgical team in intraoperative management, close postoperative follow-up and early diagnosis of complications.
