Pansitopeni ve Organomegali ile Gelen Hastada Visseral Leishmania Olgusu
Özet
Leishmaniasis, memelilerde makrofajların içinde yaşayan protozoaların neden olduğu tatarcık sineği (phlebotomus) tarafından taşınan bulaşıcı bir hastalıktır. Ülkemizde visseral leishmaniasis (VL) ve kutanöz leishmaniasis (KL) tipleri görülmektedir. Visseral leishmaniasis, bu hastalığın en ciddi klinik formudur. Kalıcı ateş, splenomegali ve pansitopeni viseral leishmaniasiste sık görülen bulgulardır. Bu vakada 18 yaşında erkek hasta kilo kaybı, ateş, gece terlemesi şikayetleri ile kliniğimize başvurdu. Hastada pansitopeni ve hepatosplenomegali saptandı. Bunun üzerine yapılan kemik iliği aspirasyon/biyopsisi ile VL tanısı konuldu. 7 gün lipozomal amfoterisin B tedavisi ile dalak ve karaciğer büyüklüğü dahil diğer semptomları geriledi. Bu vakamızda lösemi, lenfoma gibi kemik iliği tutulumu ile giden malign hastalıklar ön planda düşünülmesine rağmen ayırıcı tanıda leishmaniazis gibi enfeksiyöz hastalıkların da olabileceğine dikkat çekmek istedik.
