Allojenik Kemik İliği Nakli İle Tedavi Edilen Langerhans Hücreli Histiyositoz Olgusu
Özet
Langerhans hücreli histiyositoz (LHH), myeloid kökenli malign histiyositik bir hastalıktır. Morfolojik olarak benzedikleri epidermal langerhans hücrelerinden farklıdır. Neoplastik histiyositik hastalıklar içinde en sık görüleni langerhans hücreli histiyositozdur. LHH en sık kemikleri ve cildi etkiler. Ancak kemik iliği, karaciğer, dalak, akciğerler, merkezi sinir sistemi (MSS), hipofiz bezini ve diğer organları da tutabilir. 51 yaşında kadın hasta büyüyen lenf nodları, ateş, gece terlemesi şikayetleri ile başvurdu. Hastanın yapılan lenf nodu eksizyonel biyopsisi LHH olarak geldi. Çeşitli kemoterapiler sonrası %100 uyumlu kardeş vericisinden yapılan allojenik kemik iliği nakli ile tedavi edildi. Hastanın tanısından 10 yıl önce diyabetes insipitus hastalığı teşhisi olup LHH’nin tutulumu olarak düşünüldü. Allojenik nakil tedavisi ile hastanın desmopressin ihtiyacı da geriledi.
