Akut Promyelositik Lösemi Hastasında Tretinoin Kullanımına Bağlı Papilödem: Olgu Sunumu
Özet
Psödotümör serebri (PTC), normal beyin omurilik sıvısı (BOS) bileşiminin eşlik ettiği artmış kafa içi basıncı ile karakterize edilen bir durumdur. Genellikle birçok tıbbi durum ve ilaçla ilişkilidir. Tretinoinin (all-trans-retinoik asit; ATRA) PTC’yi indükleyebileceği bildirilmiştir. Akut promiyelositik lösemi (APL) tanısı ile takip edilen ve ATRA’ya sekonder intrakranial hipertansiyon ve papilödem gelişen 24 yaşındaki kadın vakamızı sunuyoruz. Hasta 100 mg/gün ATRA ile tedavi edildi. Tedavi protokolünün 14. gününde şiddetli baş ağrısı, çift görme ve bulantı şikâyetleri başladı. Oftalmolojik muayenesinde bilateral papilödem ve sağ gözde görme keskinliğinde azalma saptandı. Diğer fizik muayenelerinde özellik yoktu. Çekilen orbita manyetik rezonans görüntülemesi (MRG), intrakranial hipertansiyon ile uyumlu olduğu düşünülen bilateral optik sinir çevresinde BOS mesafelerinde hafif genişleme, sinir traselerinde hafif tortüozite ve glob arka yüzlerinde minimal düzleşme şeklinde raporlandı. Ayrıca çekilen kontrastlı serebral MRG, serebral diffüzyon MRG ve serebral MRG venografide anormal bulgu tespit edilmedi. Ek olarak lomber ponksiyon uygulandı ve BOS bulgusu yüksek BOS basıncı dışında normaldi. Modifiye Dandy kriterlerine göre tanı PTC’idi. ATRA kesildikten sonra hastanın semptomları azalarak kayboldu.
