Özet

Kuru göz hastalığı (DED), gözyaşı filmi homeostazının bozulmasıyla seyreden; instabilite, hiperozmolarite, inflamasyon, oküler yüzey hasarı ve nörosensoriyel değişiklikleri içeren multifaktöriyel bir tablodur. Güncel tanı yaklaşımı, semptom anketlerini (OSDI, SPEED, DEQ-5, SANDE) TBUT/NIBUT, Schirmer ve gözyaşı menisküs yüksekliği, korneokonjonktival boyanma, osmolarite, yatak başı MMP-9, meibografi ve OCT temelli ölçümlerle birleştirir. Göz kırpma dinamikleri, ilaç maruziyeti ve sistemik eşlikler alt tip ayrımını (ADDE vs EDE/MGD) ve şiddet sınıflamasını netleştirir. Tek bir ölçüt yeterli değildir; doğru sınıflama ve mekanizmaya yönelik tedavi için bütüncül sentez gereklidir.
